Subscription Billing for Online Pharmacies
How to bill prescription refills on subscription: follow the prescription, charge on shipment, stop when refills run out, and pick a tool that fits.
Article summary
TL;DR
A pharmacy subscription bills each refill the prescriber has authorized, ideally on the day it ships, and stops when refills run out or the prescription expires. Show the terms before the first order, get express consent, and let customers pause, skip, change their card or cancel online. The tool must follow the pharmacy's dispensing, not a fixed calendar date.
- Federal law allows a prescription drug to be refilled only "if such refilling is authorized by the prescriber".
- Tagada Rx rebills "the day the pharmacy ships", not on a calendar date, with a 30-day default cycle.
- Stripe found that 25% of lost subscriptions are lost to failed payments (January 2024).
- Shopify Subscriptions works only with listed gateways, and Shopify Payments excludes prescription drugs.

What makes pharmacy subscriptions different
A regular subscription ships the same product until the customer cancels. A pharmacy subscription depends on a prescription that has its own limits.
| Regular subscription | Pharmacy refill subscription | |
|---|---|---|
| What allows the next order | The customer's consent | The consent, plus an authorized refill121 U.S.C. § 353(b)(1), read November 2, 2026 |
| When it ends | When the customer cancels | When the customer cancels, refills run out or the prescription expires |
| Quantity | Chosen by the customer | Set by the prescription |
| Renewal | Automatic | Needs the prescriber when refills run out |
| Data involved | Order and payment | Order and payment; prescription data stays in the pharmacy system |

Billing ends with the prescription, not only with a cancel button.
Your subscription tool needs one input from the pharmacy system: can this refill ship? If not, no charge.
Bill on shipment, not on a date
Charging on a fixed date creates orders that are paid but can't ship: a refill not yet authorized, a prescription that expired, a pharmacist question. Billing when the pharmacy ships avoids that.
- Refill due. The tool flags the subscription a few days before the cycle date.
- Pharmacy check. The pharmacy confirms a refill is authorized and the order can be filled.
- Reminder. The customer gets a notice with the amount and the ship date.
- Ship and charge. The card is charged when the order ships.
- No refill left. The tool pauses the subscription and the customer is told to renew the prescription.
No authorized refill, no charge.
Tagada Rx, the stack Tagada runs for prescription brands, works this way: recurring orders rebill "the day the pharmacy ships" and the default cycle is 30 days2Tagada docs, Tagada Rx introduction, read November 2, 2026.
Watch out
A subscription that keeps charging after the last authorized refill leads straight to refund requests and disputes. Tie the end of billing to the prescription, not only to a cancel button.
The billing rules
A refill subscription is a recurring charge. US law and card network rules apply to it like any other.
| Rule | What it requires |
|---|---|
| ROSCA (US law) | Material terms before billing, express consent, a simple way to stop recurring charges3Restore Online Shoppers' Confidence Act, 15 U.S.C. § 8403 |
| Visa | Express consent; trial terms shown separately; reminder before a trial ends; online cancellation4Visa Core Rules and Visa Product and Service Rules, edition of April 18, 2026, tables 5-20 and 5-21 |
| Mastercard | Confirmation with terms and cancellation instructions after sign-up; online cancellation; notice before billing on cycles of six months or less5Mastercard, Subscription/recurring payments and negative option billing merchants: FAQs, 2022 |

| Refill completion | Refills shipped ÷ refills due | Gaps in prescriber renewals |
| Renewal failure rate | Failed charges ÷ renewal attempts | Card and processor issues |
| Skips and pauses | Skips ÷ refills due | Cycle too short for actual use |
