Failed Payment Recovery for Online Pharmacies
Why refill payments fail, a recovery schedule built around the ship date, card network retry rules, and messages that keep health details private.
Article summary
TL;DR
A failed refill payment is a customer you lose without them deciding to leave. Recover it in layers: refresh cards before renewal, retry soft declines within card network limits, send a private one-click card update, and cascade to another processor when one is down or times out. Decide in advance what happens to the shipment while payment is pending.
- Stripe ties 25% of lost subscriptions to failed payments (January 2024).
- 90% of recovered payments are recovered within the first 10 days after the failure, per Recurly (April 2026).
- Visa allows up to 20 attempts in 30 days for retryable declines and forbids retrying category 1 decline codes (Visa rules, October 24, 2026).
- Once a customer is lost to a payment issue, only about 5% ever resubscribe, as reported by the Merchant Risk Council.

The recovery playbook for a refill
Pharmacy refills add one decision that most subscriptions don't have: what to do with an order that is ready to ship but not paid. Set the rule once, write it in your terms, and apply it to everyone.
| When | Action |
|---|---|
| Before the refill | Run card updates; email customers whose card expires before the next charge |
| Ship day, the failure | Read the decline code; hold the shipment; send a private card update link |
| Days 1 to 10 | Retry soft declines at different times; one email and one SMS reminder |
| Processor down or timed out | Retry on a backup processor that has approved your pharmacy |
| Payment recovered | Ship the same day; the customer may be close to the end of their supply |
| Day 10 onward | Last reminder; pause the subscription instead of cancelling it |

Watch out
A long recovery can outlast the prescription. If the prescription expires or refills run out during the retries, stop charging and ask the customer to renew the prescription first.

Messages that don't expose health details
Payment reminders are read on lock screens, shared inboxes and family phones. Keep the health details out.
- No medication name in the subject line, SMS or push notification. "Your next order" is enough.
- One-click card update link, ideally without a login, that expires after use.
- Say what is on hold and until when: "Your refill is ready to ship as soon as your card is updated."
- Use the pharmacy name your customers know, the same as on the card statement.
- Stop when it's fixed: no reminder after the payment goes through.

What each message does
- Ship day: says what is on hold and until when, with no medication name.
- Day 2: a one-click card update link that expires after use.
- Day 3: the payment went through; the reminders stop.
- Your pharmacyShip dayYour next order is on holdYour refill is ready to ship as soon as your card is updated.
- Your pharmacyDay 2Your next order is waitingOne-click card update link inside.
- Your pharmacyDay 3Payment receivedYour next order ships today.
Flows: email flows for online pharmacies and SMS flows for online pharmacies.
Measure recovery
| Metric | Formula | Watch for |
|---|---|---|
| Failure rate | Failed refills ÷ refills charged | Spikes by processor or card type |
| Recovery rate | Recovered ÷ failed refills | Low: retry timing or messages |
| Time to recovery | Days from failure to payment | Over 10 days: a gap in supply |
| Involuntary churn | Subscriptions ended by failed payment ÷ active | Compare with cancellations |
| Excess retry fees | On processor statements | Any: retry logic breaks network rules |
Recurly estimates involuntary churn at 20% to 40% of total churn across subscription businesses1Recurly, Failed payment recovery: a data-based strategy, April 30, 2026. Split yours to see what recovery can win back: subscriptions and churn for online pharmacies. Backup processors are covered in payment routing for online pharmacies.
