Best Ecommerce Platform for Online Pharmacies

How to choose an ecommerce platform for an online pharmacy: payment eligibility, prescription checks, refills, ads and a sourced comparison.

Article summary

TL;DR

The best platform for an online pharmacy is the one that lets you take payment for a prescription order at all. Check five things in order: which processors accept your pharmacy, how prescriptions are checked before the card is charged, how health data stays out of the store, how refills bill, and how ads are tracked.

  • Shopify Payments does not support "prescription drugs", as of November 2026. You need a third-party provider, which Shopify charges 0.2% to 2% per order for.
  • Stripe lists online pharmacies as restricted, not prohibited: it asks for licenses and may still decline.
  • Card-not-present pharmacy sales use MCC 5912, and Mastercard requires the acquirer to register the pharmacy before processing.
  • Meta requires online pharmacies to be "actively certified with LegitScript" to advertise.
  • HIPAA does not apply to the payment step itself, under section 1179 of the Social Security Act.

This page is the hub of our online pharmacy playbook. The payment side lives in online pharmacy payment processing. The general guide, for any business: checkout optimization.

Why online pharmacies choose a platform differently

A clothing store picks a platform for its themes. An online pharmacy has four risks that most stores never face.

RiskWhat happensWhere it hits
Payment eligibilityThe platform's built-in payments exclude prescription drugs1Shopify Help Center, Shopify Payments eligibility, read November 2, 2026You cannot take a single order without a separate processor
Network registrationYour acquirer must register you as a non-face-to-face pharmacy8Mastercard Security Rules and Procedures, Merchant Edition, August 4, 2026, §9.4.3Approval takes documents, licenses and proof of legality
Health dataSome platforms refuse protected health information in the store2Shopify, Acceptable Use Policy, read November 2, 2026Prescription uploads and intake answers need a separate home
Ad restrictionsGoogle, Meta and TikTok require certification before pharmacy ads9Google Ads policy, Healthcare and medicines, read November 2, 202610Meta Advertising Standards, Drugs and pharmaceuticals, read November 2, 202611TikTok Ads policy, Healthcare and pharmaceuticals, updated September 2026NoCertification, no paid acquisition

None of these appear in a feature checklist. All four decide whether the pharmacy can sell at all.

Watch out

Shopify's Acceptable Use Policy says its platform does not support "uploading Protected Health Information subject to HIPAA"2Shopify, Acceptable Use Policy, read November 2, 2026. A prescription upload form built inside a Shopify theme is the wrong place for that data.

The 5 criteria that matter, in order

Rank platforms on these five points before you compare themes or app counts.

#CriterionWhy it matters for a pharmacyQuestion to ask the platform
1Processor choiceBuilt-in payments often exclude prescription drugs; you bring an approved processorCan I connect my own processor, and at what extra fee?
2Charge after the prescription checkThe order should not be charged before a pharmacist or clinician has approved itCan I authorize at checkout and capture later, or void?
3Health data kept out of the storeIntake answers and prescriptions are health data; payment data is not1242 U.S.C. § 1320d-8 (Social Security Act § 1179)Where do prescription documents live, and who stores them?
4Refills and subscriptionsRepeat orders drive revenue; billing must follow the pharmacy, not a calendarCan a refill bill when the order ships, and pause or skip?
5Ads and trackingAd platforms certify pharmacies and limit health data in eventsAre purchase events sent server-side, without health details?

Fig. 1Five criteria, in the order to apply them

  1. 1. Processor choiceEntry filter: bring an approved processor
  2. 2. Charge after the checkAuthorize at checkout, capture later or void
  3. 3. Health data out of the storeWhere prescriptions live, and who stores them
  4. 4. Refills and subscriptionsBill when the order ships, pause or skip
  5. 5. Ads and trackingServer-side events, no health details

Processor choice is the entry filter: fail it and nothing else matters.

The five criteria from the table above.

Criterion 1 is detailed in payment routing and backup processors. Criteria 2 and 4 are covered in checkout optimization for online pharmacies and subscription billing.

Tagada also runs on top of Shopify or WooCommerce, or headless. You can keep your store and change only the checkout, subscriptions and payments layer. Weighing the two models? Read Shopify or a sales funnel for your online pharmacy.

Prescription checks: authorize first, capture later

The cleanest pharmacy checkout does not charge the card until the prescription is approved. Card networks allow this with two steps: an authorization (a hold) at checkout, then a capture or a void.

  1. Checkout: the customer pays; the card is authorized, not charged.
  2. Review: a pharmacist or clinician checks the prescription or intake.
  3. Approved: you capture the authorization and the order ships.
  4. Declined: you void the authorization. Nothing is refunded because nothing was charged.

This avoids charging, then refunding, every rejected order. Tagada's documentation describes the same flow for Tagada Rx: approval captures the hold, a decline voids it15Tagada docs, Tagada Rx introduction, read November 2, 2026.

Watch out

A card hold is not a medical approval. Tagada's Rx documentation says the storefront copy "must make that clear"15Tagada docs, Tagada Rx introduction, read November 2, 2026. Write it next to the pay button.

Payment rules for prescription orders are on the payment side, in how to sell prescription drugs on subscription.

Refills and subscriptions

Refills are where an online pharmacy keeps its customers. They are also where a platform choice is hardest to undo.

  1. Bill on shipment, not on a fixed date

A refill that bills before the pharmacy ships creates "charged but never shipped" orders. Tagada Rx rebills "the day the pharmacy ships"15Tagada docs, Tagada Rx introduction, read November 2, 2026.

  2. Let customers pause or skip

    Instead of cancelling.

  3. Card tokens belong to the processor

If the processor closes your account, saved cards may stay behind. Plan this before you need it: switching payment processor without losing cards.

  4. Follow the renewal rules

Card networks and US law set their own rules for recurring billing. Details: subscriptions and churn for online pharmacies.

Ads and tracking: certification comes first

An online pharmacy cannot advertise like a regular store. Each platform has its own gate.

PlatformWhat it requires from an online pharmacy
Google Ads (US)Accreditation by LegitScript or NABP, plus Google certification9Google Ads policy, Healthcare and medicines, read November 2, 2026
MetaActive LegitScript certification and written permission; 18+ targeting; US, Canada or New Zealand only10Meta Advertising Standards, Drugs and pharmaceuticals, read November 2, 2026
TikTok Ads (US)Proof of certification by a third party "such as" NABP or LegitScript11TikTok Ads policy, Healthcare and pharmaceuticals, updated September 2026
  1. Google Ads (US)LegitScript or NABPGoogle certification
  2. MetaActive LegitScript certificationWritten permission; 18+ targeting; US, Canada or New Zealand only
  3. TikTok Ads (US)Third-party certification such as NABP or LegitScriptProof of certification

Your platform then needs to send purchase events from the server, without health details in the payload. Setup: Meta and TikTok ads for online pharmacies and server-side tracking. Ad rules in full: can you advertise prescription drugs on Facebook, Google and TikTok?

Which setup fits your pharmacy model

The right platform depends less on your size than on how you operate.

Your modelWhat you need mostSetup that fits
Licensed pharmacy adding online ordersA processor that registers you under MCC 59127Visa Merchant Data Standards Manual, October 2026 edition, prescription intake, refillsYour store (Shopify, WooCommerce or headless) plus a checkout and subscription layer that takes your processor
Prescription brand without its own pharmacyClinicians, a licensed pharmacy partner, payments, certificationAn all-in-one stack such as Tagada Rx
OTC catalog plus prescriptionsCatalog tools for OTC, a separate gated flow for prescription itemsA catalog store for OTC, a gated checkout for prescriptions
Paid ads to a single offerFast checkout, server-side tracking, certificationA funnel and checkout layer (sales funnels for online pharmacies)

Example

A licensed mail-order pharmacy that already fills prescriptions locally ranks criteria 1, 2 and 4 first. A new brand that sells treatment plans through Meta ads ranks certification, criterion 5 and an all-in-one stack first.

What to check before you commit

Run this list with any platform's sales team. A clear answer on each point saves a migration later.

  1. Processor: which processors can I connect, and what extra fee applies?
  2. Eligibility: whose restricted list applies to prescription orders on my store?
  3. Capture: can I authorize at checkout and capture after the prescription is approved?
  4. Health data: where are prescriptions and intake answers stored, and under which agreement?
  5. Refills: can renewals bill on shipment, with pause and skip?
  6. Card portability: if I change processor, do my refill customers keep paying?
  7. Tracking: are purchase events sent server-side, with no health data?
  8. Exit: can I export customers, orders and subscriptions?

A clear answer on each point saves a migration later.

Frequently asked questions

Can you sell prescription drugs on Shopify?

You can build the store on Shopify, but Shopify Payments lists "prescription drugs" among the products it cannot support. You need a third-party provider that accepts pharmacies, and Shopify charges 0.2% to 2% per order on top, by plan. Its policy also excludes uploads of HIPAA-covered health information. Details: can you sell prescription drugs on Shopify?

Does Stripe accept online pharmacies?

Stripe lists "Online pharmacies" as a restricted business, not a prohibited one. Its FAQ answers "Maybe" and says it must follow card network rules for pharmaceutical merchants. Pharmacies are only approved in the US, Canada, the EU, Australia, the UK and Japan. Approval can be modified or revoked later.

Is HIPAA a problem for my payment processor?

Not for the payment step. Section 1179 of the Social Security Act says HIPAA does not apply to authorizing, processing or settling a payment for health care, including by card. Intake answers and prescriptions are a different matter: keep them out of payment data and out of platforms that refuse them.

Do I need LegitScript to run an online pharmacy store?

Not to build the store. You need it, or NABP accreditation, to advertise on Google, and LegitScript specifically on Meta. Card networks ask the acquirer for proof that the pharmacy is legal, and a certification is the simplest proof. See MCC 5912.

Which platforms bill refills automatically?

Of the platforms in this guide, WooCommerce needs a paid extension, BigCommerce needs an app or a gateway, and Shopify relies on Shopify Subscriptions or apps. Tagada includes subscriptions with trials, cycles and retries; Tagada Rx rebills on the day the pharmacy ships.
