Order Bumps for Online Pharmacies
What an online pharmacy can offer as an order bump, what it cannot, how the bump works with a prescription review hold, and how to measure it.
Article summary
TL;DR
An online pharmacy order bump is one small, related item a patient can add with a checkbox before paying: a non-prescription product, a supply, or a service such as faster shipping. It is never a prescription medication, which needs its own prescription and review. Decide in advance what happens to the bump if the prescription review is declined.
- Visa bans, for every merchant, products that "claim or imply a similar efficacy as prescription drugs" (Visa Core Rules, October 2026). Watch your bump copy.
- The FDA warns patients against sites that sell prescription medicine without a prescription. A prescription item cannot be a one-click add-on.
- Stripe lets you capture less than the authorized amount; "a partial capture automatically releases the remaining amount".
- No public study gives order bump take rates for pharmacies. Vendor figures on AOV lifts are unsourced; measure your own.

This guide is part of our hub on the best ecommerce platform for online pharmacies. The general guide, for any business: order bumps: what to offer, where to put them, how to measure them. For brands rather than pharmacies, Tagada Rx bundles storefront, doctors, pharmacy and payments, open by application.
What you can offer as a bump
A good bump is cheap next to the main order and obviously useful with it. Checkout Page puts it simply: keep it "well below the price of the main product so accepting it feels like an easy yes"4Checkout Page, Order bumps, read November 2, 2026.
| Bump type | Example | Check before you offer it |
|---|---|---|
| Shipping upgrade | Faster or temperature-controlled delivery | Your carrier can actually deliver it |
| Supplies | Pill organizer, travel case, storage pouch | Not a medical device that needs a prescription |
| Non-prescription products | A relevant over-the-counter item | NoHealth claims in the bump copy; state rules on the product |
| Pharmacist service | A scheduled consultation with your pharmacist | Your license and staffing cover it |
| Delivery protection | Coverage for lost or damaged parcels | Clear price and terms |

Keep to one bump. A second offer competes with the first and slows a checkout patients already find longer than usual. More on what makes an offer work: order bump.
What you cannot offer
Prescription medications
They need a valid prescription and pharmacist review. Offering them as a one-click add-on is selling without a prescription, the FDA's first warning sign2FDA, BeSafeRx frequently asked questions, content current as of September 21, 2020.
More medication than prescribed
A "bigger supply" option belongs in the main product choice, within what the prescription allows, not in a bump.
Products that imply drug-like efficacy
Visa bans these for all merchants1Visa Core Rules, edition of October 24, 2026, section 1.9.2.1.
Health claims
No promise of an outcome in the bump headline or description.
Bumps targeted on health answers
Do not use intake answers to choose the offer, and do not send them to marketing tools.
Restricted drug terms in paid traffic flows
If the bump appears on pages reviewed by Google, its policy on prescription drug terms applies there too5Google Ads Help, Healthcare and medicines policy, read November 2, 2026.
The bump and the prescription review
If you authorize the card at checkout and capture after review, the bump rides on the same authorization. You need a rule for a declined review.
| Setup | On approval | On declined review | Fits |
|---|---|---|---|
| Bump inside the hold | Capture order and bump | Void everything; patient pays nothing | Bumps that only make sense with the medication |
| Partial capture | Capture order and bump | Capture the bump only; the rest is released3Stripe Docs, Place a hold on a payment method, read November 2, 2026 | Bumps the patient wants anyway, such as a supply item |

Bump inside the hold

Void everything; patient pays nothing
Partial capture

Capture the bump only; the rest is released
Void everything unless the patient wants the bump anyway.
Most pharmacies should void everything on a declined review: it is simpler to explain and avoids a surprise charge. If you use partial capture, say so next to the checkbox.
Example
"Add a travel pill case for $6. Charged only if your order is approved." One line, one price, one condition.
Hold and capture in detail: checkout optimization for online pharmacies.
Set it up
- Pick one item that most patients on a given product would use, with no prescription needed.
- Price it low relative to the order.
- Write one line that says what it is and its price; add the hold condition.
- Place it under the order summary, before the pay button, unchecked by default.
- Map it to your fulfillment and to the void or capture rule above.
- Test it against no bump: see A/B testing for online pharmacies.
One item, one line, unchecked by default.
Measure it
| Metric | Formula | Watch for |
|---|---|---|
| Bump take rate | Orders with bump ÷ orders | Very low: wrong item or price |
| Checkout completion | Orders ÷ checkouts started | Drops when the bump is added |
| Margin per approved order | (Order + bump margin) ÷ approved orders | Bump margin eaten by shipping |
| Bump refunds and tickets | Bump-related contacts ÷ bumps sold | Confusion about the hold |
Bumps after payment are a different tool: see one-click upsells for online pharmacies and our 1-click upsell guide.
