How to Increase AOV for an Online Pharmacy
Raise an online pharmacy's order value without pushing medication: supply length, OTC add-ons, shipping thresholds, memberships and the rules.
Article summary
TL;DR
An online pharmacy raises order value around the prescription, not by pushing it. The quantity of medication is set by the prescriber. What you control is the rest of the order: the supply length the prescription allows, non-prescription add-ons, a free shipping threshold, a paid membership and combined household orders. Keep every add-on optional and clearly priced.
- Stripe prohibits businesses that "use high-pressure upselling". An upsell must be a clear, optional offer.
- Visa bans products that "claim or imply a similar efficacy as prescription drugs" for every merchant. Watch how add-ons are described.
- Free items offered to Medicare beneficiaries can count as illegal inducements, with an exception for retailer coupons on equal public terms.
- Post-purchase upsells average 4.7% acceptance across all sectors (ReConvert, 2023 data); optimized stores reach 16.2% (Zipify, July 2026).
- No source publishes an average order value for online pharmacies with a method, as of November 2026. Measure your own.

This guide is part of our playbook on the best ecommerce platform for online pharmacies. The general guides, for any business: order bumps and one-click upsells.
What you can and cannot add to a prescription order
The prescription decides the medication and its quantity. A one-click offer cannot add a prescription item the customer has no prescription for.
| Add-on | OK to offer? | Why |
|---|---|---|
| Longer supply of the same prescription | YesIf the prescription allows it | The prescriber and pharmacist decide; you only show the option |
| Non-prescription essentials (pill organizers, thermometers, first aid, travel cases) | Yes | Ordinary retail products |
| Over-the-counter products | YesWith neutral descriptions | NoWording that implies prescription-level effects2Visa Core Rules and Visa Product and Service Rules, edition of October 24, 2026, §1.9.2.1 |
| Faster or discreet shipping | Yes | A service, priced clearly |
| Membership (shipping, refill reminders, support) | Yes | A service; check inducement rules if you serve Medicare patients342 U.S.C. § 1320a-7a, (a)(5) and (i)(6), read November 2, 2026 |
| Another prescription item | No | Requires a valid prescription and a pharmacist review |
Fig. 1What can go on a prescription order
OK to offer
- Longer supply of the same prescription, if the prescription allows it
- Non-prescription essentials: pill organizers, thermometers, first aid, travel cases
- Over-the-counter products, with neutral descriptions
- Faster or discreet shipping, priced clearly
- Membership, after checking inducement rules if you serve Medicare patients
Not OK
- Another prescription item: it requires a valid prescription and a pharmacist review
The prescription decides the medication and its quantity.
Watch out
Never describe an OTC add-on as an alternative to a prescription medication. Visa prohibits products that "claim or imply a similar efficacy as prescription drugs, controlled substances, or recreational/street drugs"2Visa Core Rules and Visa Product and Service Rules, edition of October 24, 2026, §1.9.2.1. That rule applies to every merchant, whatever your processor.
6 levers for an online pharmacy
| Lever | How it works | Effect on AOV | Risk |
|---|---|---|---|
| Supply length | Offer the longer supply the prescription allows | High | None if the prescription allows it |
| Household orders | Add another family member's refill to the same shipment | Medium | Each refill needs its own prescription |
| Order bump | One small non-prescription item at checkout | Low to medium | Low |
| Post-purchase upsell | One optional offer after payment | Medium | Low if relevant, high if pushy1Stripe, Prohibited and restricted businesses, last updated September 22, 2026, read November 2, 2026 |
| Free shipping threshold | Set above your current median order | Low to medium | None |
| Membership | Annual fee for shipping, reminders and support | Medium over the year | Inducement rules342 U.S.C. § 1320a-7a, (a)(5) and (i)(6), read November 2, 2026 |

Use them together, test them one at a time.
The lever to avoid: blanket discounts on prescription items. They train customers to wait, and price promotions on prescription drugs raise legal questions that depend on your state and your payers.
Supply length and refills
The biggest lever in a pharmacy is usually the one you do not sell: how much each order covers and how often it renews.

Bill refills when the order ships
A charge before shipment creates orders that are paid but not sent. Tagada Rx rebills "the day the pharmacy ships"7Tagada docs, Tagada Rx introduction, read November 2, 2026.
Group refills
For customers with several prescriptions, so one shipment carries them all.

Add-ons at checkout and after payment
Add-ons work in a pharmacy when they solve a practical problem the order creates: storing, organizing, measuring, traveling.

- One offer, clearly priced, then the confirmation page. Stripe prohibits high-pressure upselling1Stripe, Prohibited and restricted businesses, last updated September 22, 2026, read November 2, 2026.
- Only non-prescription items in one-click offers.
- No health claims in the offer copy: describe what the product is, not what it does to the body.
- Never block checkout behind an add-on screen.
Setup: order bumps for online pharmacies and one-click upsells.
Free shipping threshold and memberships
Shipping is where online pharmacies compete with the counter. A threshold raises the basket; a membership turns shipping into a reason to stay.
- Take your median order, after discounts, over the last 90 days.
- Set the free shipping threshold above it.
- Suggest the item that fills the gap in the cart: an organizer, a travel case, an OTC staple.
- Offer a membership that includes shipping to customers who order every month.
Set the threshold above your current median order.
Membership design: memberships and VIP clubs for online pharmacies.
Rules on discounts and inducements
Discounts look harmless. In a pharmacy, they meet laws that ordinary stores never see.
- Medicare patients. Federal law penalizes offering "remuneration" likely to influence a Medicare beneficiary to choose a provider342 U.S.C. § 1320a-7a, (a)(5) and (i)(6), read November 2, 2026. Remuneration includes "transfers of items or services for free or for other than fair market value"342 U.S.C. § 1320a-7a, (a)(5) and (i)(6), read November 2, 2026.
- The retailer exception. "Coupons, rebates, or other rewards from a retailer" are excepted if offered on equal terms to the general public and not tied to other items or services342 U.S.C. § 1320a-7a, (a)(5) and (i)(6), read November 2, 2026.
- "Free" offers. Under FTC rules, an item is not "free" if its conditions are hidden or the price of what must be bought was raised416 CFR 233.4, Bargain offers based upon the purchase of other merchandise.
- Processor rules. Stripe prohibits high-pressure upselling1Stripe, Prohibited and restricted businesses, last updated September 22, 2026, read November 2, 2026.
Watch out
This page is not legal advice. If any of your customers' prescriptions are paid by Medicare or a state program, have a healthcare lawyer review every discount, gift and membership before launch.
Check the margin
Illustrative figures. A one-month refill earns you $20 of margin after fulfillment. Shipping costs you $8. An organizer sells for $12 and costs $4.
| Order | Margin on items | Shipping cost | Contribution |
|---|---|---|---|
| One-month refill | $20 | $8 | $12 |
| One-month refill + organizer | $28 | $8 | $20 |
| Three-month supply (if prescribed), one shipment | $60 | $8 | $52 |
| Three monthly refills, three shipments | $60 | $24 | $36 |

Fewer shipments, more contribution.
A longer supply, when the prescription allows it, saves two shipments. The add-on lifts a single order by two thirds of its contribution.
