Bundles and Quantity Breaks for Online Pharmacies: Pricing and Discounts

What bundles and quantity breaks can and cannot do in an online pharmacy: supply length, non-prescription add-ons, free shipping and the legal limits.

Article summary

TL;DR

In an online pharmacy, the prescription sets the quantity, so classic quantity breaks ("buy 3, save 20%") do not apply to prescription medications. What you can price is the supply length the prescription allows (30 or 90 days), non-prescription add-ons, and a free shipping threshold. If Medicare or a state program may pay, federal law limits discounts.

  • The FDA tells patients a safe online pharmacy "Requires a prescription from a licensed healthcare professional". Quantity follows the prescription, not the offer.
  • Federal law penalizes offering Medicare or state health program beneficiaries "remuneration" likely to influence where they buy covered items.
  • Retailer coupons and rewards are exempt only if offered to the general public on equal terms and not tied to items those programs pay for.
  • A 2023 pricing model built around free shipping thresholds improved profit in a numerical study on data from a leading e-pharmacy in China.
  • 40% of US shoppers who abandoned a cart cite extra costs such as shipping, the top reason after browsing (Baymard, updated September 22, 2025).

Watch out

A discount that pushes patients to buy more medication than prescribed is not a pricing tactic. Never offer it.

Supply length: the pharmacy's quantity break

The one "quantity" choice that works for prescription medications is how much supply ships at once, within the prescription.

  1. Offer 30 and 90 days

When the prescriber has written for it. Show the price per month next to each option.

  2. Make it a product choice, not a bump

Supply length belongs in the main selection, reviewed with the prescription.

  3. Bill refills when they ship

Patients pay for what leaves the pharmacy.

  4. State refill terms before payment

Price, frequency and how to cancel, with express consent (ROSCA)615 U.S.C. § 8403, Restore Online Shoppers' Confidence Act.

Example

"30-day supply: $X per month. 90-day supply: $Y, or $Y÷3 per month, ships every 90 days while your prescription is valid." Put your own prices in; keep the per-month figure visible.

Non-prescription add-ons and free shipping

Bundles and quantity breaks work as usual on products that need no prescription.

  1. Pick related items: storage, supplies, relevant over-the-counter products.
  2. Keep claims out: Visa bans products that "claim or imply a similar efficacy as prescription drugs" for every merchant5Visa Core Rules, edition of October 24, 2026, section 1.9.2.1. Tagada follows Visa and Mastercard rules and the laws that apply to payments.
  3. Set a free shipping threshold just above your typical order, so one add-on clears it. The 2023 study priced bundles around free shipping thresholds and customer budgets3Li, Liu, Luo and Zhu, Interactive bundle pricing strategy for online pharmacies, Transportation Research Part E, vol. 177, 2023.
  4. Show shipping early: extra costs are the top reason US shoppers abandon a checkout4Baymard Institute, Cart abandonment rate statistics and reasons for abandonment, updated September 22, 2025.

The rule that matters most applies when federal health programs may pay.

If your pharmacy is cash-pay only, ask counsel how these rules apply to you before running any prescription discount.

Frequently asked questions

Can an online pharmacy offer quantity discounts on prescription medications?

No. The prescription sets the quantity, and the FDA tells patients a safe pharmacy requires a prescription. What you can do is offer the supply lengths the prescription allows, such as 30 or 90 days, and show the price per month for each.

Can I bundle prescription and non-prescription products?

Yes, as separate lines on the same order: the prescription item still needs its review, and the other items carry no health claims. If the prescription review is declined, decide whether the add-ons ship alone or the whole order is cancelled, and say so at checkout.

Are pharmacy discount codes legal?

Often, but check first. If Medicare or a state health program may pay for the order, federal law limits rewards that influence where beneficiaries buy. Retailer coupons offered to the general public on equal terms, and not tied to reimbursed items, are exempt. State rules may add limits.

Does a free shipping threshold work for a pharmacy?

Yes. It applies to the whole order and nudges patients to add a non-prescription item. Shipping costs are the top reason shoppers abandon a checkout, so show the threshold early, on the product page and in the cart.
