Memberships and VIP Clubs for Online Pharmacies

Membership models for an online pharmacy, benefits that need no medical claim, the billing rules for a recurring fee, and how to set it up.

Article summary

TL;DR

A pharmacy membership is a recurring fee in exchange for benefits such as lower cash prices, free shipping on refills or extra service. It never replaces a prescription: every medication still needs one. Bill it like any subscription, with the price, renewal date and cancellation path shown before the customer agrees, and keep the fee separate from the medication orders.

  • Amazon's RxPass charges Prime members "a flat $5 monthly fee" for "over 50 commonly prescribed generic medications", in 48 states.
  • A Memphis membership pharmacy earned its revenue from a $30 synchronization fee per fill instead of drug margins (J Am Pharm Assoc, 2023).
  • Stripe prohibits "negative option membership clubs" with unclear or hidden pricing.
  • ROSCA requires clear terms before billing, express consent and a simple way to stop recurring charges.

This guide is part of our playbook on the best ecommerce platform for online pharmacies. The general guide, for any store: subscription retention and cancellation flows. Refill subscriptions themselves are in subscription billing for online pharmacies.

Membership or refill subscription?

Both bill on a schedule. They sell different things, and customers judge them differently.

MembershipRefill subscription
What the customer pays forAccess: prices, shipping, serviceThe medication itself, shipped on a cycle
Needs a prescriptionNoBut every medication bought with it doesYesFor every shipment
Charged whenMonthly or yearly, on a fixed dateWhen the refill ships
Stops whenThe customer cancelsThe customer cancels, or refills run out
Main riskCustomers who forget they pay itCharging for a refill that can't ship

Many pharmacies run both: a membership for the benefits, and refill subscriptions for each prescription. Brands that sell treatment plans without running a pharmacy can launch on a telehealth stack with storefront, doctors, pharmacy and payments.

Three membership models

Published examples show three ways to build a pharmacy membership.

Example

A cash-pay mail-order pharmacy charges $49 a year. Members get free shipping on every refill and member cash prices. Non-members pay shipping per order. The fee is billed once a year, with a reminder before renewal.

Benefits you can offer without medical claims

Keep the benefits about price, convenience and service. Never promise a health result.

  1. Member cash prices

On medications, shown next to the regular price.

  2. Free or faster shipping

    On refills.

  3. Refill synchronization

    So several prescriptions ship together.

  4. Priority pharmacist messaging

    Or a dedicated support line.

  5. Discounts on non-prescription items

    In your store.

  6. Early renewal reminders

    Before a prescription runs out.

Watch out

"better results for members" or "stay healthy with our club" are health claims. Describe what the member gets, not what it does for their health.

Billing rules for a membership fee

A membership fee is a recurring charge. The same rules apply as for any subscription.

Free-trial memberships add their own rules: trial offers for online pharmacies. Recurring billing rules for prescription orders: how to sell prescription drugs on subscription.

Set it up in your store

  1. Create the membership as its own product, separate from any medication.
  2. Show the terms at sign-up: price, billing cycle, next charge date, how to cancel.
  3. Get a separate "Join" click, not a pre-ticked box in the medication checkout.
  4. Apply the benefits automatically: member prices and free shipping when the member logs in.
  5. Send the confirmation email with the terms and the cancellation link.
  6. Remind before renewal, especially for annual fees.
  7. Let members manage it themselves: pause, cancel and update their card in a customer portal.

Seven steps, from product to self-serve cancellation.

Retention offers at cancellation: subscriptions and churn for online pharmacies. Emails that support the membership: email flows for online pharmacies.

Frequently asked questions

What is a pharmacy membership program?

A recurring fee, monthly or yearly, that gives members benefits such as cash prices, free shipping or extra service. It does not replace a prescription or insurance. Amazon's RxPass, for example, charges Prime members $5 a month for a list of eligible generics, outside insurance.

Is a pharmacy membership the same as a prescription subscription?

No. A membership charges for access to benefits, on a fixed schedule. A refill subscription charges for the medication, usually when it ships, and stops when the prescription runs out. Many pharmacies offer both, billed separately.

Can a pharmacy charge a membership fee for discounted medications?

Many do, as cash-price clubs, but check your state's pharmacy rules and your contracts with insurers first. On the payment side, the fee is a recurring charge: show its terms, get express consent and offer a simple way to cancel.

How do I keep members from disputing the fee?

Make it hard to forget. Show the terms at sign-up, send a confirmation with the cancellation link, remind before each annual renewal, and use a billing descriptor members recognize. Stripe prohibits membership clubs with unclear or hidden pricing.
