How to Sell Prescription Drugs on Subscription
How an online pharmacy sells prescription refills on subscription: authorize, capture on approval, rebill on ship, and Visa, Mastercard, state rules.
Article summary
TL;DR
You can sell prescription refills on subscription if every shipment is backed by a valid prescription and dispensed by a licensed pharmacy. Bill in the pharmacy's order: authorize the card at checkout, capture once the prescription is approved, and charge each refill when it ships. Show price, frequency and cancellation terms before the card field; allow online cancellation.
- A legitimate online pharmacy "Requires a prescription from a licensed healthcare professional", according to the FDA.
- A card authorization for an online payment is valid for 7 days at Visa and Mastercard for customer-initiated payments, per Stripe's documentation read November 2, 2026.
- Visa requires a reminder at least 7 days before a trial ends and online cancellation (rules of April 18, 2026).
- New York's Attorney General secured $400,000 from an online medication provider over hidden subscription terms on August 24, 2026.
- Since March 31, 2026, Google lets certified US online pharmacies promote prescription subscriptions in Merchant Center.

Can you sell prescription drugs on subscription?
Yes, with conditions. The subscription bills the refill; it never replaces the prescription.
A valid prescription
It covers each shipment, and the pharmacist checks it before dispensing1FDA, BeSafeRx frequently asked questions, content current as of September 21, 2020.
A licensed pharmacy
In the patient's state, it dispenses the order1FDA, BeSafeRx frequently asked questions, content current as of September 21, 2020.
The prescription sets the limits
The number of refills and the end date. When it runs out, billing stops until a new one is approved.

Running a telehealth brand rather than a pharmacy? The consultation side has its own rules: telehealth subscriptions and checkout.
Bill in the order the pharmacy works
Charging before a prescription is approved creates refunds and disputes when it's declined. Authorizing first avoids both.
- Checkout: collect the card and place an authorization (a hold) for the first order.
- Prescription review: the prescriber or pharmacist approves or declines.
- Approved: capture the authorized amount.
- Declined: cancel (void) the authorization. Nothing is charged, so there's nothing to refund.
- Refills: charge each one when the pharmacy ships it, not on a fixed calendar date.
How long you can wait between steps 1 and 3 depends on the card brand.
| Card brand | Authorization valid for (online, customer-initiated) | If you don't capture in time |
|---|---|---|
| Visa | 7 days | The hold is released and the payment is canceled |
| Mastercard | 7 days | Same |
| American Express | 7 days | Same |
| Discover | 7 days | Same |

Watch out
Some card statements show a hold like a charge. Stripe notes this "can sometimes confuse customers"2Stripe Docs, Place a hold on a payment method, read November 2, 2026. Say at checkout that the card is authorized now and charged only once the prescription is approved.

How Tagada helps
A billing order that follows the pharmacy
Tagada Rx runs this billing order for brands selling prescription treatment online, with storefront, doctors, pharmacy and payments in one stack.
- Hold, capture on approval: "The card is only captured once a doctor approves." If the doctor declines, the hold is released and the patient pays nothing
- Rebill on ship: recurring treatments rebill the day the pharmacy ships; the default cycle is 30 days (Rx documentation)
- Refills, on autopilot, with pharmacy routing to a licensed US pharmacy
- Customer portal: patients can skip, pause, change frequency, update cards or cancel themselves (portal documentation)
- Open by application to a selected group of brands; Tagada reviews every application
Terms are on the Tagada Rx page. Tagada's docs add one rule for your copy: the hold is not clinical approval, and the storefront must say so.
