Chargebacks for Online Pharmacies: Thresholds, Prevention and Alerts Setup

Visa and Mastercard chargeback thresholds for online pharmacies in 2026, why patients dispute, and the payment setup and alerts that prevent it.

Article summary

TL;DR

Visa and Mastercard both flag merchants at 1.5% in 2026, but an online pharmacy's acquirer watches much earlier: Visa makes it check pharmacy disputes daily. Pharmacy disputes come from card holds read as charges, orders charged then refused, refills billed before shipping and unrecognized descriptors. Capturing only after approval, billing refills on ship day and alerts prevent most of them.

  • Visa's US threshold is 1.5% of settled transactions, fraud plus disputes, since April 1, 2026, with at least 1,500 cases a month.
  • Mastercard's Excessive Chargeback Merchant level starts at 1.5% and 100 chargebacks, as reported by Chargeback Gurus (October 21, 2025).
  • From day 31, the acquirer of a Tier 1 merchant, such as a pharmacy, compares its daily disputes with its normal activity (Visa Core Rules, October 24, 2026).
  • Disputes resolved through a pre-dispute solution are excluded from Visa's count.
  • Card statements don't always tell an authorization from a charge, which "can sometimes confuse customers" (Stripe).

The thresholds in 2026

Example

4,000 settled orders in a month, 50 disputes and 20 fraud reports. Visa ratio = 70 ÷ 4,000 = 1.75%. That's over 1.5% but far under Visa's 1,500-case minimum, so no Visa program. Your acquirer still sees it the next morning.

Why pharmacies are watched earlier

The thresholds above are the floor. Pharmacy acquirers carry extra network obligations, so their own tolerance is lower.

  1. Visa, daily

From day 31 of a Tier 1 merchant, the acquirer tracks volume, average ticket, transactions and disputes every day against normal activity4Visa Core Rules and Visa Product and Service Rules, edition of October 24, 2026, section 10.4.5.2.

  2. Mastercard, yearly

The acquirer re-verifies the pharmacy's legality at least every 12 months5Mastercard Security Rules and Procedures, Merchant Edition, August 4, 2026, sections 8.8 and 9.4.3; a dispute pattern weighs on that review.

  3. Mastercard BRAM

Its Business Risk Assessment and Mitigation program "investigates Merchants referred to Mastercard as potentially engaging in illegal or brand-damaging Transactions"5Mastercard Security Rules and Procedures, Merchant Edition, August 4, 2026, sections 8.8 and 9.4.3.

Prevention that works

  1. Authorize, verify, then capture. Nothing is charged before the prescription is approved: how to require a prescription before checkout.
  2. Void refused orders instead of refunding them; the hold disappears from the statement.
  3. Bill refills on ship day, not on a calendar date, so no one pays for an order that hasn't left.
  4. Remind before each refill on cycles of 6 months or less (Mastercard) and before trials end, at least 7 days ahead (Visa)8Visa Core Rules and Visa Product and Service Rules, edition of October 24, 2026, tables 5-19 and 5-209Mastercard, Subscription/recurring payments and negative option billing merchants: FAQs, 2022.

  6. A recognizable descriptor: pharmacy name and phone. See billing descriptor.

Alerts setup

Alerts warn you of a dispute before it's filed, so you can refund and keep it out of the count. Visa excludes disputes resolved through a pre-dispute solution1Visa, Acquirer Monitoring Program fact sheet, 2025 v2.

  • Ask your processor which alert services it connects: Visa Rapid Dispute Resolution, Ethoca, Verifi.
  • Set auto-refund rules for low-value orders, and review high-value ones by hand.
  • Cancel the refill at the same time as the refund, or the next charge becomes the next dispute.
  • Track alerts by cause: they show which step of the flow leaks.

Frequently asked questions

What is the chargeback threshold for online pharmacies in 2026?

Visa monitors US merchants at 1.5% of settled transactions, fraud reports plus disputes, with at least 1,500 cases a month, since April 1, 2026. Mastercard starts at 1.5% and 100 chargebacks. A pharmacy's acquirer reviews disputes daily, so its own limit is usually lower.

How do I calculate my chargeback ratio?

For Visa: fraud reports plus disputes in the month, divided by settled card-not-present transactions the same month. For Mastercard: chargebacks in the month, divided by the previous month's transactions. Track both by product and by refill versus first order.

Why do online pharmacies get chargebacks?

Mostly from the payment flow, not fraud: holds read as charges, orders charged then refused, refills billed before shipping, and descriptors patients don't recognize. Capturing after approval, billing on ship day and clear descriptors fix most of them.

Should I fight every pharmacy chargeback?

No. Respond when your records prove consent and delivery, or when Visa's Compelling Evidence 3.0 applies to a fraud claim. Accept cases where the patient cancelled or the hold wasn't voided, and fix that step.

Do chargebacks affect my LegitScript or NABP accreditation?

We found no network rule that ties accreditation to dispute ratios. The rules do keep the acquirer watching: daily under Visa Tier 1 and a legality review every 12 months under Mastercard. A bad pattern can cost you the merchant account first.
