Chargeback Alerts for Online Pharmacies

How Ethoca, Verifi RDR, CDRN and Order Insight keep an online pharmacy's disputes out of its ratio, and how to set refund rules for prescriptions.

Article summary

TL;DR

Chargeback alerts warn you that a cardholder is disputing a payment before it becomes a chargeback, so you can refund or answer first. Verifi (Visa) offers Order Insight and RDR; Ethoca (Mastercard) offers Ethoca Alerts and Consumer Clarity. For an online pharmacy on a high-risk pharmacy code, every dispute kept out of the ratio protects the account.

  • Disputes resolved through RDR or Ethoca Alerts "don't count towards your overall dispute rates", per Stripe.
  • Visa's VAMP ratio excludes disputes resolved through a pre-dispute solution.
  • Visa puts online pharmacies (MCC 5122 and 5912) in its high-integrity risk group; acquirers monitor these merchants daily from day 31 (Visa rules, October 24, 2026).
  • Mastercard requires acquirers to register non-face-to-face pharmacies before processing (edition of August 4, 2026).
  • One alert vendor lists $15 per RDR or CDRN alert and $29 per Ethoca alert in the US.

Why disputes weigh more on an online pharmacy

Because your acquirer answers to Visa and Mastercard for you. A pharmacy selling online is not a standard merchant for the card networks: it is registered, watched, and reviewed.

RuleWhat it means for your disputes
Visa high-integrity risk registration (MCC 5122, 5912), unless accredited by NABP or another body Visa recognizesYour acquirer must be approved by Visa to board you (Visa)3Visa Core Rules and Visa Product and Service Rules, edition of October 24, 2026, sections 1.9.5.1 and 10.4.5.2
Daily monitoring from day 31Volume, average ticket, transaction count and disputes are tracked every day (Visa §10.4.5.2)3Visa Core Rules and Visa Product and Service Rules, edition of October 24, 2026, sections 1.9.5.1 and 10.4.5.2
Mastercard registration of non-face-to-face pharmaciesProof of legality, re-verified at least every 12 months (Mastercard §9.4.3)4Mastercard Security Rules and Procedures, Merchant Edition, August 4, 2026, sections 8.8 and 9.4.3
Mastercard BRAM programInvestigates merchants "potentially engaging in illegal or brand-damaging Transactions" (Mastercard §8.8)4Mastercard Security Rules and Procedures, Merchant Edition, August 4, 2026, sections 8.8 and 9.4.3
Visa VAMPDisputes and fraud reports counted together; 1.5% "Excessive" level in the US (Visa)2Visa, Visa Acquirer Monitoring Program fact sheet, 2025

A rising dispute count shows up in that daily monitoring before it reaches any monthly threshold. Alerts keep resolved cases out of the count. The full registration picture: online pharmacy merchant account compliance.

Tagada follows Visa and Mastercard rules and the laws that apply to payments.

Where pharmacy disputes come from

Most disputes on prescription orders start with a payment the patient didn't expect or doesn't recognize.

CauseWhat the patient tells the bankAlert that helpsFix the cause
Charged before the prescriber decided"I was charged and never got anything"RDR, EthocaAuthorize at checkout, capture only after approval
Refill charged before it ships"Not received"Ethoca, CDRNCharge on ship day, send tracking
Pharmacy name on the statement differs from the brand"I don't recognize this"Order Insight, Consumer ClarityBrand in the descriptor
Renewal after the patient stopped treatment"Cancelled recurring"RDRNotice before each charge, online cancellation
Stolen card"Fraud"Order Insight with CE 3.0 on repeat patients3DS on risky first orders

The first two causes are about timing. A checkout that holds the card and captures on approval removes them: prescription-gated checkout.

RDR rules for a pharmacy

Start narrow: a broad rule refunds disputes you could have won5Chargeback.io, RDR vs CDRN vs Ethoca, read October 27, 2026 (alert vendor).

CaseSuggested ruleWhy
Low amountsAuto-resolveFighting costs more than the refund; Stripe's example is fraud disputes under 10 USD1Stripe Docs, How dispute prevention works, read November 2, 2026

Watch out

A refund at the alert stage rarely brings the product back. Georgia, for example, makes it unlawful for a pharmacy to accept for refund "any unused portion of a drug which has been previously dispensed", except where law permits6Georgia Comp. R. & Regs. 480-16-.03, Return of Previously Dispensed Drugs or Devices, effective February 9, 2016. Count every resolved alert as a full loss of the order cost.

What Order Insight shares with the bank

Order Insight sends the issuer the order details Stripe holds: product description, quantity, amounts, delivery address, email, IP address and tracking1Stripe Docs, How dispute prevention works, read November 2, 2026. The bank's agent can show them to the cardholder.

  1. Product descriptions

Decide with your counsel how prescription items are labeled in payment data. A neutral label such as "Prescription order #1042" still identifies the order.

  2. Store details

The store name and support phone help the patient recognize the charge1Stripe Docs, How dispute prevention works, read November 2, 2026.

  3. CE 3.0

With 2 earlier orders sharing the IP address and an email or delivery address, the issuer must block a Visa card-absent fraud dispute1Stripe Docs, How dispute prevention works, read November 2, 2026.

Card payment processing sits outside HIPAA for the payment itself; what you send beyond that is a separate question: HIPAA and payments.

Costs

Prices vary by reseller and processor; Stripe lists its own on its pricing page1Stripe Docs, How dispute prevention works, read November 2, 2026. CDRN and Ethoca can fire on the same payment: deduplicate by order so you don't refund twice.

Set up in order

  1. Fix timing: capture on prescriber approval, charge refills on ship day.
  2. Fix recognition: brand name in the descriptor, order details on every payment.
  3. Turn on Order Insight and Consumer Clarity so banks see the order first.
  4. Add RDR with narrow rules for low amounts and cancelled renewals.
  5. Add Ethoca Alerts for Mastercard.
  6. Review monthly with your acquirer's dispute report: causes, products, states.

Prevention first, then alerts from the narrowest rules to the widest.

Frequently asked questions

Do chargeback alerts work for online pharmacies?

Yes. Order Insight, RDR, CDRN and Ethoca work on any card-not-present merchant whose processor supports them. Disputes resolved through RDR or Ethoca don't count in your dispute rates, which matters when your acquirer watches pharmacy merchants daily.

Do refunded alerts count in my chargeback ratio?

Not when resolved through RDR or Ethoca, per Stripe, and Visa's VAMP ratio excludes disputes resolved through a pre-dispute solution. Ask your acquirer how its own internal limits count them.

Can a pharmacy take back medication after refunding an alert?

Usually not. Many state pharmacy rules forbid accepting dispensed drugs back; Georgia makes it unlawful to accept unused dispensed drugs for refund except where law permits. Treat each refunded alert as a lost order.

What's the difference between Ethoca and Verifi?

Ethoca is a Mastercard solution, Verifi a Visa solution. Ethoca offers Ethoca Alerts and Consumer Clarity; Verifi offers RDR, Order Insight and CDRN. RDR resolves Visa disputes automatically under rules you set.

How much do chargeback alerts cost?

One vendor lists $15 per RDR or CDRN alert and $29 per Ethoca alert in the US. Prices vary by reseller, and every resolved alert is also a refund.
