Checkout Optimization for Online Pharmacies

How to build an online pharmacy checkout: the prescription step, holding the card until approval, refill terms, and the fixes that cut abandonment.

Article summary

TL;DR

An online pharmacy checkout converts when it is short, shows the full cost early, lets patients pay as guests with Apple Pay or Google Pay, and states refill terms above the pay button. The difference is the prescription step: authorize the card at checkout, let the pharmacist review, then capture on approval or void on decline.

  • Extra costs drive 40% of checkout abandonment among US shoppers, forced accounts 18% and declined cards 10% (Baymard, September 2025).
  • The average checkout has 11.3 form fields; Baymard says most sites need 8 (2024).
  • An online card authorization on Visa or Mastercard is valid for 7 days for customer-initiated payments, per Stripe.
  • On Shopify, apps that customize the information, shipping and payment pages "are available only to Shopify Plus".
  • Refill terms must be disclosed before billing information is collected, with express consent (ROSCA).

The prescription step: three ways to handle it

Every order for a prescription medication needs a valid prescription before it ships. Where that step sits changes your checkout.

ApproachHow it worksTrade-off
Upload before paymentThe patient attaches a prescription; checkout is blocked until a file is presentAdds friction; the file still needs review
Prescription sent by the prescriberThe patient names their prescriber; the pharmacy receives the prescription electronicallySmoothest for the patient; review happens after checkout
Review after authorizationThe patient pays by authorization; a pharmacist reviews before captureFast checkout; you must handle declined reviews cleanly

On WooCommerce, plugins add the first approach. One, Prescription for WooCommerce, blocks checkout on the server until a file is attached and puts the order "On hold" for a pharmacist to approve or reject6WordPress.org, Prescription for WooCommerce (DevDiggers), last modified October 23, 2026. It holds the order, not the money: unless you change your payment setup, the card is charged at checkout.

  2. Prescription sent by the prescriberReview happens after checkoutWith a review step, authorize then capture
  3. Review after authorizationA pharmacist reviews before captureAuthorized at checkout, captured or voided after review

Charge, hold or refund

The order of events at payment decides how many refunds and disputes you get.

MethodWhat happens on a declined reviewCost
Charge at checkoutYou refund the patientFees, support tickets, risk of a dispute
Authorize, then captureYou void the authorization; the patient is not chargedLowest, if review fits the window

A void costs nothing; a refund costs fees, tickets and sometimes a dispute.

Illustrative example.

Most pharmacies with a review step should authorize then capture. Stripe lists a 7-day validity window for customer-initiated online authorizations on Visa and Mastercard; after that, the funds are released3Stripe Docs, Place a hold on a payment method, authorization validity windows, read November 2, 2026. Tagada follows Visa and Mastercard rules and the laws that apply to payments.

Watch out

Some card statements do not distinguish a hold from a charge3Stripe Docs, Place a hold on a payment method, authorization validity windows, read November 2, 2026. Say it next to the button: "Your card is authorized now and charged only if your order is approved."

Refills at checkout

Refills are where a pharmacy checkout is most regulated, and where most of the revenue comes from.

  1. Clear terms before payment

Price, how often the refill ships, when it is charged and how to cancel. ROSCA requires the terms before billing information is collected715 U.S.C. § 8403, Restore Online Shoppers' Confidence Act.

  2. Express consent

An unchecked box or a clear choice for the refill plan, never pre-selected715 U.S.C. § 8403, Restore Online Shoppers' Confidence Act.

  3. Charge on shipping

Tell patients each refill is charged when it ships, not on a calendar date.

  4. Within the prescription

A refill plan cannot outlast the prescription. Say what happens when it ends.

Example

"Refills ship every 30 days while your prescription is valid. Each refill is charged on the day it ships. Cancel anytime from your account." placed directly above the button.

How Tagada helps

A prescription checkout that holds, then captures on approval

  • Tagada Rx runs the prescription checkout for you: storefront, doctors, pharmacy and payments in one stack, LegitScript included. "Approve captures the hold. Decline voids it." Money never leaves the card if the clinician does not prescribe, and refills rebill the day the pharmacy ships9Tagada docs, Tagada Rx introduction, read November 2, 2026. US patients only; open by application to a selected group of brands.
  • One-page checkout with Apple Pay and Google Pay, standalone or on Shopify, WooCommerce or headless.
  • Order bumps, one-click upsells and A/B tests on the checkout.
  • Routing across TagadaPay and processors such as Stripe, NMI and Airwallex, with one logic; cascade to another processor when one is down or times out, never after a bank decline.
  • 3DS2, device intelligence and a rules engine against fraud.

Frequently asked questions

How do I require a prescription before checkout?

On WooCommerce, a prescription plugin can block checkout until a file is attached and hold the order for pharmacist approval. On Shopify, no confirmed app does this, per a 2024 merchant thread. Or skip the upload: take an authorization at checkout and receive the prescription from the prescriber before capture.

Should I charge the card before the pharmacist approves?

Better not. Authorize the card at checkout and capture only after approval; a declined review then ends with a voided authorization instead of a refund. Keep review within the authorization window, 7 days for most online card payments per Stripe.

Can Shopify hold checkout until a prescription is approved?

Not natively. Apps that customize Shopify's information, shipping and payment pages are available only on Shopify Plus, and a merchant asking for this exact feature in 2024 found no app that did it. Many pharmacies use an outside checkout instead.

How should I show a refill plan at checkout?

State the price, how often refills ship, when each is charged and how to cancel, right above the pay button, and ask for express consent with an unchecked box. ROSCA requires the terms before billing details are collected.

How can I reduce checkout abandonment on my pharmacy site?

Show the full cost early, allow guest checkout, cut the form to about 8 fields, add Apple Pay and Google Pay, and explain the hold. Extra costs cause 40% of abandonments and forced accounts 18%. Then track declined payments, which cause another 10%.
