Express Checkout for Online Pharmacies

Which wallets an online pharmacy can offer, what Apple, Google, Shopify and PayPal require, how HSA and FSA cards fit in a prescription checkout.

Article summary

TL;DR

An online pharmacy can usually offer Apple Pay and Google Pay, as long as its processor accepts the pharmacy and supports the wallets. Shop Pay comes with Shopify Payments, which excludes prescription drugs, and PayPal requires pre-approval for prescription sales. HSA and FSA cards often need their own path. The wallet speeds up payment; the prescription check still happens.

  • Apple Pay's web rules ban "non-legally prescribed controlled substances", not pharmacies as such.
  • Google Pay's API policy, effective October 21, 2026, bars healthcare services unless you hold a relevant license.
  • Shop Pay "is included as part of Shopify Payments", and Shopify Payments does not support prescription drugs.
  • PayPal lists prescription items under activities that require its approval (policy updated October 29, 2022).
  • Offering at least one accelerated wallet "can increase lower funnel conversion rates by upward of 50%", per Shopify and BCG in June 2023.

Which express payment methods an online pharmacy can offer

Each wallet has its own rules on top of your processor's. Here is what each one says, in its own words.

MethodWhat its rules say about pharmaciesWhat it means for you
Apple PayBans "Illegal drugs or non-legally prescribed controlled substances" and sites that violate any law1Apple, Acceptable Use Guidelines for Apple Pay on the Web, read November 2, 2026Possible for a licensed pharmacy, through a processor that supports it
Google Pay"We do not allow the APIs to be used for the provision of healthcare content and services", unless you hold a relevant license2Google, Google Pay and Wallet APIs Acceptable Use Policy, effective October 21, 2026Be ready to show your pharmacy license
Shop Pay"Included as part of Shopify Payments"3Shopify Help Center, Shop Pay, read November 2, 2026; Shopify Payments excludes "prescription drugs"4Shopify Help Center, Shopify Payments eligibility, read November 2, 2026Not available through Shopify Payments for prescription orders

Your processor is the first filter. A wallet only works if the processor behind your checkout has approved the pharmacy and enabled that wallet. Processor rules: online pharmacy payment processing.

Where express checkout sits in a prescription flow

A wallet skips typing. It must not skip the prescription step. Place the wallet buttons after the steps that only the pharmacy can do.

  1. Prescription or intake first. The customer uploads a prescription, or the order is linked to one, before payment.
  2. Wallet at payment. Apple Pay or Google Pay fill the card, name and address in one tap.
  3. Authorize, don't capture yet. The card is held while a pharmacist checks the order.
  4. Capture or void. Approved orders are captured and ship; declined orders are voided, so nothing is refunded.

The wallet skips typing, never the prescription step.

Tagada's Rx documentation describes the same pattern: approval captures the hold, a decline voids it10Tagada docs, Tagada Rx introduction, read November 2, 2026. It also says the storefront must make clear that the hold is not clinical approval10Tagada docs, Tagada Rx introduction, read November 2, 2026. Put that line next to the wallet buttons.

Brands that would rather not build this flow can apply to Tagada Rx, a stack with storefront, doctors, pharmacy and payments.

Watch out

Express buttons on a product page let customers pay before any intake. For prescription items, show them at the payment step, not on the product page.

HSA and FSA cards

Many pharmacy customers want to pay with a health savings or flexible spending card. These cards follow their own rules.

  1. FSA and HRA cards need an IIAS at most pharmacies

SIGIS, which runs the industry standard, says drug stores and pharmacies must implement an Inventory Information Approval System, unless they meet the IRS "90% rule" on qualifying sales6SIGIS, Program comparison FAQ (IIAS and the IRS 90% rule), read November 2, 2026.

  2. Web-based sellers of prescription drugs are named

SIGIS lists "mail order vendors and web-based vendors that sell prescription drugs" among merchants that need an IIAS6SIGIS, Program comparison FAQ (IIAS and the IRS 90% rule), read November 2, 2026.

  3. Wallets often can't carry them

Some health brands tell customers to "not select an Express checkout method" when paying with HSA or FSA9Solawave Help Center, What payment methods do you accept?, read November 2, 2026.

  4. Your processor decides

Ask whether it supports IIAS data, such as the prescription amount field, before you promise HSA and FSA payment.

Set it up

  1. Confirm your processor accepts your pharmacy and supports Apple Pay and Google Pay.
  2. Register your domain with Apple Pay through your processor.
  3. Keep your license ready for Google Pay's review2Google, Google Pay and Wallet APIs Acceptable Use Policy, effective October 21, 2026.
  4. Apply to PayPal separately if you want it5PayPal, Acceptable Use Policy, last updated October 29, 2022.
  5. Decide on HSA and FSA: IIAS support, or a clear message that these cards go through card entry.
  6. Test refills and upsells on wallet orders: the saved card must work for the next charge.

How Tagada helps

Wallets in a checkout that holds until approval

Tagada Rx gives prescription brands "storefront, doctors, pharmacy and payments in one stack"11Tagada Rx, tagada.io/rx, read November 2, 2026, with LegitScript included. Payment follows the review: hold at checkout, capture on approval, rebill on ship day11Tagada Rx, tagada.io/rx, read November 2, 2026. It serves US patients and is open by application to a selected group of brands: see a telehealth stack with storefront, doctors, pharmacy and payments.

For the store itself, the Tagada ecommerce OS includes:

  • Apple Pay and Google Pay in a one-page checkout
  • One-click upsells and order bumps that keep working after payment
  • Subscriptions with retries for refill orders
  • Server-side tracking to Meta, Google and TikTok

It runs standalone, on Shopify, on WooCommerce or headless.

Tagada follows Visa and Mastercard rules and the laws that apply to payments.

Frequently asked questions

Can an online pharmacy accept Apple Pay?

Yes, in most cases. Apple's web rules ban "non-legally prescribed controlled substances" and illegal activity, not licensed pharmacies. You add Apple Pay through a processor that has approved your pharmacy and supports the wallet, and you verify your domain with Apple.

Can I use Shop Pay on a Shopify pharmacy store?

Not through Shopify Payments. Shopify says Shop Pay "is included as part of Shopify Payments", and Shopify Payments does not support prescription drugs. A pharmacy on Shopify uses a third-party provider and offers the wallets that provider supports.

Can customers pay with an HSA or FSA card in express checkout?

Often not. Several health brands ask customers to leave express checkout to pay with HSA or FSA. For FSA and HRA cards, SIGIS says pharmacies need an IIAS unless they meet the 90% rule. Check with your processor before promising it.

Does PayPal work for prescription orders?

Only with PayPal's approval. Its Acceptable Use Policy lists "the sale of any product(s) requiring a prescription or prescription dispensing services" among activities that require pre-approval. Without it, those transactions are not allowed.
