Payment Methods for Online Pharmacies

Which payment methods an online pharmacy can offer: cards, wallets, HSA and FSA cards, PayPal pre-approval, BNPL rules, and what works with refills.

Article summary

TL;DR

Offer cards first, with Apple Pay and Google Pay on top, and accept HSA and FSA cards if your processor supports them. PayPal requires pre-approval for prescription items. Buy now, pay later is mostly closed: Afterpay prohibits prescription drugs and Klarna treats prescription pharmacies as restricted. Pick methods that allow a hold, since the charge waits for the prescription.

  • 9% of US shoppers who abandoned a checkout cite not enough payment methods (Baymard).
  • PayPal lists "the sale of any product(s) requiring a prescription or prescription dispensing services" among activities requiring approval.
  • Afterpay prohibits "Prescription drugs, regulated products, illegal drugs" (updated July 15, 2026).
  • Klarna lists "Prescription pharmacies" as a restricted business; approval is case by case.
  • ACH debits don't support a separate authorization and capture at Stripe; cards, PayPal and Klarna do.

HSA and FSA cards

Many patients want to pay prescriptions from a health account. The card works only if your store is set up for it.

A BNPL button switched on in your processor's dashboard doesn't mean the BNPL provider approved a pharmacy. Get written approval before you show it, or the account behind it can be closed.

Frequently asked questions

What payment methods should an online pharmacy accept?

Cards, Apple Pay and Google Pay first, plus HSA and FSA cards if your processor supports them. Add PayPal once it has approved prescription items. Baymard finds 9% of abandoning US shoppers cite too few payment methods.

Can an online pharmacy accept PayPal?

Only with PayPal's approval. Its policy lists "the sale of any product(s) requiring a prescription or prescription dispensing services" among activities requiring approval, and activities requiring approval are prohibited without it.

Can patients pay for prescriptions with Afterpay or Klarna?

Not with Afterpay, which prohibits prescription drugs. Klarna lists prescription pharmacies as restricted, so approval is case by case and can be revoked at its discretion. Ask in writing before adding either.

Can patients use HSA or FSA cards on my pharmacy site?

Yes, if your processor supports them and your store meets the IRS route for pharmacies: an inventory approval system or the 90% rule. Stripe offers both routes, without guaranteeing every card will be approved.

Do Apple Pay and Google Pay work for refills?

Usually. Wallet payments run through your card processor, and saved credentials can renew off-session without 3DS. Check that your subscription system supports wallet credentials for recurring charges.
