Do PayPal and Square Allow Prescription Drugs?
PayPal requires pre-approval for prescription items; Square's US terms exclude internet pharmacies. The exact clauses and what happens to your funds.
Article summary
PayPal approval, Square noTL;DR
PayPal allows prescription sales only with its prior approval; without it, they break PayPal's Acceptable Use Policy. Square's US terms exclude internet, mail-order and telephone-order pharmacies outright. Neither publishes how it approves pharmacies, and both can hold your funds if they decide you're out of bounds.
- PayPal lists "Prescription Items" among activities requiring approval: "The sale of any product(s) requiring a prescription or prescription dispensing services" (policy updated October 29, 2022).
- PayPal prohibits any activity "that requires pre-approval without having obtained said approval".
- Square's US terms exclude "internet/mail order/telephone order pharmacies or pharmacy referral services" (updated July 30, 2026).
- PayPal can hold funds "for up to 180 days" (User Agreement updated September 14, 2026).

What PayPal requires
PayPal's Acceptable Use Policy has a table titled "Activities Requiring Approval". Row 8, "Prescription Items", covers "The sale of any product(s) requiring a prescription or prescription dispensing services"1PayPal Acceptable Use Policy, updated October 29, 2022 (US page read via Wayback Machine copy, 2026).
Two other clauses frame it:
- Without approval, it's prohibited. You may not use PayPal for transactions that "relate to transactions involving any activity that requires pre-approval without having obtained said approval"1PayPal Acceptable Use Policy, updated October 29, 2022 (US page read via Wayback Machine copy, 2026).
- Some products stay out regardless. The policy also excludes "narcotics, steroids, certain controlled substances or other products that present a risk to consumer safety"1PayPal Acceptable Use Policy, updated October 29, 2022 (US page read via Wayback Machine copy, 2026).
PayPal doesn't publish what it checks before approving a pharmacy. Older press reports describe certification requirements, but they date from the 2000s and don't reflect a current policy. Expect to show the same file an acquirer asks for: state licenses, your prescription process and a certification if you have one.
Watch out
A PayPal business account opens in minutes, but that doesn't mean prescription sales are approved. Approval for this category is a separate request.
Why Square says no
Square's US Payment Terms list industries Square doesn't support. Item (8) reads: "internet/mail order/telephone order pharmacies or pharmacy referral services (where fulfillment of medication is performed with an internet or telephone consultation, absent a physical visit, including re-importation of pharmaceuticals from foreign countries)"3Square, Payment Terms (US), sections 3 and 12, updated July 30, 2026.
| Your model | Covered by Square's exclusion? |
|---|---|
| Online pharmacy shipping prescriptions | Yes |
| Prescription orders taken by phone and mailed | Yes |
| Referral site sending patients to a pharmacy | Yes("pharmacy referral services") |
| Online consultation followed by medication shipped | Yes("absent a physical visit") |
The clause targets remote dispensing. It doesn't describe a pharmacy counter where the patient picks up in person. If that's your business, confirm with Square in writing before you rely on it.
What happens to your money
Both providers can keep funds when they decide an account breaks their rules.
| Provider | What the terms say |
|---|---|
| PayPal | Can hold funds "for up to 180 days if reasonably needed to protect against the risk of liability or if you have violated our Acceptable Use Policy"2PayPal User Agreement, updated September 14, 2026 |
| PayPal | Reserve example: "your reserve could be set at 10% and held for a 90-day rolling period"2PayPal User Agreement, updated September 14, 2026 |
| Square | "May defer payout or restrict access to your Proceeds, temporarily or indefinitely"3Square, Payment Terms (US), sections 3 and 12, updated July 30, 2026 |
Fig. 2Three ways your money can stay behind
PayPal hold
Funds held for up to 180 days to protect against liability or after an Acceptable Use Policy breach.
PayPal reserve
10% held for a 90-day rolling period, in PayPal's example.
Square deferral
Payout deferred or access to proceeds restricted, temporarily or indefinitely.
A reserve runs while you sell. A hold can take the whole balance.
Example
A pharmacy sells $60,000 a month through PayPal without approval. PayPal limits the account. Under the 180-day clause, a month of sales can stay locked for half a year while refills keep shipping2PayPal User Agreement, updated September 14, 2026.
Where to go instead
Pharmacies that can't use PayPal or Square have two routes.
- Stripe, after approval. Stripe lists online pharmacies as restricted, not prohibited, and reviews each one6Stripe, Restricted and prohibited businesses, updated September 22, 2026. Details: does Stripe allow prescription drugs?
- A dedicated merchant account. An acquirer underwrites you and registers you with Visa and Mastercard as a pharmacy4Visa Core Rules and Visa Product and Service Rules, October 24, 2026, section 1.9.5.15Mastercard Security Rules and Procedures, Merchant Edition, August 4, 2026, section 9.4.3. Steps: how to get an online pharmacy merchant account approved.
Both routes start with an underwriting review.
Whichever route, the networks ask your acquirer for proof of legality, such as "accreditation by a recognized third party"5Mastercard Security Rules and Procedures, Merchant Edition, August 4, 2026, section 9.4.3. Visa exempts NABP-accredited pharmacies from its high-risk registration4Visa Core Rules and Visa Product and Service Rules, October 24, 2026, section 1.9.5.1.
Tagada follows Visa and Mastercard rules and the laws that apply to payments.
