International Payments for Online Pharmacies
Can an online pharmacy sell abroad? Licenses, EU distance-selling rules, acquirer country, high-risk cross-border codes, Stripe's countries and ads.
Article summary
TL;DR
Licenses decide before payments do. A US pharmacy must be licensed in each state it ships to, and every other country applies its own law to the medication it receives. To open a market, you need an acquirer in your pharmacy's country, Strong Customer Authentication for European cards, and a processor accepting pharmacies there. Stripe approves them in six markets.
- Stripe: "Pharmacies will only be approved for merchants in the US, Canada, the EU, Australia, the UK, and Japan".
- In the EU, medicines sold at a distance must comply with the law of the Member State of destination, and the site must show the EU common logo.
- A Visa acquirer "must accept Transactions only from a Merchant Outlet within the Acquirer's Country of Domicile", unless Visa approves otherwise.
- Outside the US, Visa treats MCC 5122 and 5912 as high integrity risk for non-domestic card-absent transactions (October 2026).
- FDA lists "the medicine will be shipped from a foreign country" among the signs of an unsafe online pharmacy.

This guide is part of our hub on online pharmacy payment processing. The general guide, for any business: international payments: what changes. If you sell prescription treatment as a brand, Tagada Rx puts storefront, doctors, pharmacy and payments in one stack, by application.
The product rule decides first
A processor reads where you ship before it reads your checkout. Prescription medications are licensed product by product and place by place.
| Market | Before you sell |
|---|---|
| United States | A pharmacy license in each state you ship to; federal law states it for controlled substances, and states require it in practice (21 U.S.C. § 831(b))121 U.S.C. § 831(b) |
| European Union | Authorized to supply the public at a distance in your Member State, notified to its regulator; products comply with the destination country's law; common logo on every page offering medicines (Art. 85c)3Directive 2001/83/EC, Article 85c, inserted by Directive 2011/62/EU (text as published on legislation.gov.uk, as of December 31, 2020) |
| EU Member States | National law may still prohibit the distance sale of prescription medicines (Art. 85c(1))3Directive 2001/83/EC, Article 85c, inserted by Directive 2011/62/EU (text as published on legislation.gov.uk, as of December 31, 2020) |
| Other markets | Check the national regulator before the first shipment (Each regulator) |
Watch out
The FDA tells US patients that a safe online pharmacy is licensed in their state, requires a prescription and has a US address, and that "FDA does not have regulatory oversight of prescription medicine from outside the legitimate U.S. drug supply chain"2FDA, BeSafeRx: Frequently asked questions, content current as of September 21, 2020. A US pharmacy that adds foreign supply, or a foreign one that ships to US patients, looks exactly like the profile networks and processors refuse.
Square, for example, excludes pharmacies whose fulfillment involves "re-importation of pharmaceuticals from foreign countries"11Square, General Terms, Payment Terms §3 (US), last updated July 30, 2026. The legal basics by market: is it legal to sell prescription drugs online?

| Declines | Foreign issuers see an unfamiliar merchant on a high-risk code (Reducing payment declines) |

Markets where Stripe approves pharmacies: US, Canada, EU, Australia, UK, Japan
0.678%
Added to a Visa cross-border sale in the merchant's currency (TD, 2026)
1.13%
Added when the sale is in another currency (TD, 2026)
In practice, each country is a separate pharmacy operation: local license, local acquirer, local currency. Serving foreign patients from a US account is a cross-border sale on a high-risk code, with the fees and declines that come with it.
Fig. 1Where Stripe approves pharmacies: the product rule, then the payment rule
United States
Product: a pharmacy license in each state you ship to. Payment: an acquirer in the US, the pharmacy's country.
European Union
Product: authorization in your Member State, the common logo, and any national ban on distance sale. Payment: a local acquirer, SCA so 3DS when required.
Canada, Australia, UK, Japan
Product: check the national regulator before the first shipment. Payment: an acquirer in the pharmacy's country.
Product rules come first: a payment setup can't fix a missing license.
