How to Start and Scale an Online Pharmacy

Start an online pharmacy step by step: model, state licenses, NABP or LegitScript, payments, first channel, then what breaks when you scale.

Article summary

TL;DR

Start with the model, not the website. Decide whether you will run a licensed pharmacy or build a brand on top of a licensed partner. Then secure licenses for your state and every state you ship to, a certification such as NABP or LegitScript, and a processor that accepts pharmacies. Only then build the store and pick one acquisition channel.

  • Federal law requires an online pharmacy that dispenses controlled substances to comply with licensing rules in each state it ships from and to.
  • The FDA tells consumers to buy only from pharmacies licensed in their state and the state where the pharmacy operates.
  • LegitScript costs $975 to apply and $2,150 a year per website; NABP's Digital Pharmacy Accreditation lasts 3 years.
  • Shopify Payments excludes prescription drugs and Square prohibits internet pharmacies; Stripe restricts them.
  • Amazon and TikTok Shop prohibit prescription drugs for third-party sellers, so your own site is the channel.

Summary of the table above.

Stage 1: choose your model

The first decision is who holds the pharmacy license.

Example

A brand that wants to sell treatment plans without a pharmacy of its own can use an all-in-one stack. Tagada Rx routes orders to "a licensed US pharmacy" and works with "US-licensed clinicians, licensed in the patient's state"14Tagada Rx, tagada.io/rx, read November 2, 2026.

Licenses and the pharmacy

Licensing is state by state. Plan it before anything else.

  1. Home state and destination states

For controlled substances, federal law requires compliance with licensing rules "in each State from which it, and in each State to which it, delivers"121 U.S.C. § 831, Additional requirements relating to online pharmacies. States set their own rules for other prescriptions; check each board.

  2. What consumers check

The FDA tells them a safe pharmacy "Requires a prescription", "Provides a physical address in the United States", is licensed in their state and yours, and "Has a state-licensed pharmacist available"3FDA, BeSafeRx frequently asked questions, content current as of September 21, 2020.

  3. Controlled substances

Dispensing them online requires "a valid prescription", which in principle supposes at least one in-person medical evaluation221 U.S.C. § 829(e), Controlled substances dispensed by means of the Internet. Pharmacies that dispense them must post specific disclosures on their homepage121 U.S.C. § 831, Additional requirements relating to online pharmacies.

Watch out

This page is not legal advice. Licensing, ownership and pharmacist requirements vary by state. Work with a pharmacy lawyer and each state board before you ship.

Certification: NABP or LegitScript

Certification is not a license. It proves your legitimacy to payment and ad platforms.

How Tagada helps

Tagada Rx at launch, your own payment stack at scale

Tagada Rx is the simplest path for a brand that doesn't want to run a pharmacy: "Storefront, doctors, pharmacy and payments in one stack. You bring the brand and the ads"14Tagada Rx, tagada.io/rx, read November 2, 2026. Payments and LegitScript are included, and the card is captured only once a clinician approves. It serves US patients only and is open by application; details on the Tagada Rx page.

For your own store:

  • Routing: TagadaPay routes each transaction "to the processor most likely to approve it, retry on decline"15TagadaPay, tagadapay.io, read November 2, 2026
  • Portability: your cards and subscription data stay portable: "Switch processors without re-onboarding customers"15TagadaPay, tagadapay.io, read November 2, 2026
  • Review: regulated verticals go through enhanced review
  • The Tagada OS adds checkout, upsells, subscriptions and server-side tracking, standalone or on Shopify and WooCommerce

Tagada follows Visa and Mastercard rules and the laws that apply to payments.

Frequently asked questions

How do I start an online pharmacy?

Choose your model (your own licensed pharmacy or a brand on a licensed partner), get licensed in your state and the states you ship to, obtain NABP accreditation or LegitScript certification, find a processor that registers online pharmacies, then build the store and one acquisition channel. Start with licenses: everything else depends on them.

Do I need a license in every state I ship to?

For controlled substances, federal law requires compliance with state licensing in each state you ship from and to. For other prescriptions, states set their own rules, and the FDA tells consumers to check that a pharmacy is licensed in their state. Check each state board.

How do I start a mail-order pharmacy?

The same way: a licensed pharmacy, licenses or permits for destination states, a pharmacist-in-charge, and a process for verifying prescriptions. Online ordering adds certification, a processor that accepts pharmacies, and a checkout that charges after approval.

Can I sell prescription drugs on Amazon or Shopify?

Not on Amazon: it prohibits prescription drugs for third-party sellers. On Shopify you can build the store, but Shopify Payments excludes prescription drugs, so you need a third-party processor. See can you sell prescription drugs on Shopify?

Do I need LegitScript to start an online pharmacy?

Not to be licensed. But Meta requires LegitScript for pharmacy ads, and card networks expect proof of legality that a certification makes simpler. NABP accreditation is the alternative for Google and TikTok.
