How to Start an Online Pharmacy: Licenses, Certification and Payments

The launch order for a US online pharmacy: licenses, certification, merchant account, card network registration, and the payment mistakes to avoid.

Article summary

TL;DR

Start with the licenses, not the website. Form the company, license the pharmacy in its home state and every state you'll ship to, get certified by LegitScript or NABP, then apply for a merchant account that underwrites pharmacies. Stripe restricts online pharmacies, Shopify Payments and Square won't take them, and PayPal requires pre-approval. Plan for an early reserve.

  • Federal law requires online pharmacies that dispense controlled substances to follow state licensing rules in each state they ship from and to.
  • Harbor Compliance reports that most state pharmacy license applications "are accepted or denied within three months".
  • LegitScript charges $975 to apply and $2,150 a year per website, as published in January 2026.
  • Stripe approves pharmacies only in "the US, Canada, the EU, Australia, the UK, and Japan", after extra due diligence.
  • The FDA cites NABP: about 5% of roughly 35,000 online pharmacies complied with US pharmacy laws (FDA content of 2020).

Two ways to start

You can run the pharmacy yourself, or build a brand that relies on a licensed pharmacy partner. The licenses, the merchant account and the risk sit in different places.

How Tagada helps

The brand model, with pharmacy and payments included

If you choose the brand model, Tagada Rx gives you a telehealth stack with storefront, doctors, pharmacy and payments. You bring the brand and the ads.

  • Pharmacy routing to a licensed US pharmacy, with US-licensed clinicians licensed in the patient's state
  • Payments and LegitScript included, on a Tagada-hosted domain that holds the certification
  • Hold, capture on approval, rebill on ship for first orders and refills
  • Headless option: already have a storefront? Plug the same stack in through the SDK
  • Your data is yours to export
  • Open by application to a selected group of brands, for US patients only; Tagada reviews every application

Terms are on the Tagada Rx page.

Frequently asked questions

How much does it cost to open an online pharmacy?

TRUiC estimates $50,000 to $500,000 in total, including $2,000 to $5,000 for licensing and compliance. Certification adds $975 per website to apply to LegitScript and $2,150 a year after approval. Budget cash for a processor's reserve too.

Do I need a pharmacy license in every state?

You need a license where the pharmacy operates, and you must follow the licensing rules of each state you ship to. Harbor Compliance describes a mail-order license being required in the other states. Check each state board of pharmacy before shipping there.

Can I use Stripe or Shopify Payments for an online pharmacy?

Shopify Payments lists prescription drugs as ineligible. Stripe treats online pharmacies as restricted: it may approve them after extra due diligence, only in certain countries, and can revoke approval. Most pharmacies use a processor that underwrites the category.

Do I need LegitScript to start an online pharmacy?

No law requires it, but it's the usual proof for payments and ads. Visa exempts NABP-accredited pharmacies from its high-risk registration, and Mastercard accepts accreditation by a recognized third party as proof of legality. NABP accreditation is the other common route.

How long does it take to launch an online pharmacy?

Licensing sets the pace: Harbor Compliance reports most state applications are decided within three months, and you may need several states. Certification and merchant account underwriting come after. Neither LegitScript nor the card networks publish a timeline.
