Online Pharmacy Processing Fees and Reserves
What an online pharmacy pays to take cards: published rates, rolling reserves, card network registration and certification costs, what to ask first.
Article summary
TL;DR
An online pharmacy pays more than a standard retailer to take cards. Published rates run from "as low as" 2.95% to 6% or 7% for small pharmacies, a rolling reserve often holds part of each sale, and card networks add registration and monitoring costs. Most also pay for a certification such as LegitScript. Get every term in writing.
- Instabill reports that pharmacies above $1 million a month "generally pay between 4-5%", and small or new ones "between 6-7%".
- Visa and Mastercard make your acquirer register an online pharmacy (MCC 5912 or 5122) before it takes card-not-present payments.
- Mastercard can fine an acquirer up to $10,000 for processing an unregistered pharmacy, as of its August 4, 2026 rules.
- LegitScript charges a $975 application fee and $2,150 a year per website, as published in 2026.
- Stripe says a reserve "may be required indefinitely" in rare cases.

This guide is part of our hub on online pharmacy payment processing. The general guide, for any business: payment processing fees for high-risk merchants. If you sell prescription treatment as a brand, Tagada Rx puts storefront, doctors, pharmacy and payments in one stack, by application.
What an online pharmacy pays
Your bill has six parts. Only the first two come from your processor's price sheet.

What processors publish
Few processors publish pharmacy pricing. These are their own figures, read on November 2, 2026, not Tagada's.

Costs only pharmacies carry
Pharmacies sit in Visa's highest-risk tier for card-not-present sales. That adds steps, and some of them cost money.
| Requirement | What the rule says | Cost to you |
|---|---|---|
| Visa registration | Your acquirer must register merchants under MCC 5122 or 5912 as "high integrity risk"3Visa Core Rules and Visa Product and Service Rules, October 24, 2026, section 1.9.5.111Visa Merchant Data Standards Manual, October 2026 (MCC 5912 and 5122) | Priced into your rate; the acquirer carries the program |
| Visa exemption | Registration "does not apply" if the pharmacy is accredited by NABP "or other regulatory body recognized by Visa"3Visa Core Rules and Visa Product and Service Rules, October 24, 2026, section 1.9.5.1 | The certification's own fee |
| Mastercard registration | "An Acquirer first must register the Merchant with Mastercard" for non-face-to-face pharmaceutical sales4Mastercard Security Rules and Procedures, Merchant Edition, August 4, 2026, sections 9.2.1 and 9.4.3 | An "annual registration fee" per merchant; the 2026 manual does not publish the amount4Mastercard Security Rules and Procedures, Merchant Edition, August 4, 2026, sections 9.2.1 and 9.4.3 |
| Proof of legality | Mastercard accepts a legal opinion or "accreditation by a recognized third party", re-checked at least every 12 months4Mastercard Security Rules and Procedures, Merchant Edition, August 4, 2026, sections 9.2.1 and 9.4.3 | Legal fees or certification |
| LegitScript | $975 application fee per website, then $2,150 a year after approval5LegitScript, Healthcare Certification FAQ and 101 Guide, January 2026, read November 2, 2026 | $3,125 in year one per site |
Tagada follows Visa and Mastercard rules and the laws that apply to payments.
Watch out
A certification is a cost per website. Three storefronts means three applications. Plan this before you split brands across domains.
The full rule set: online pharmacy merchant account compliance. What MCC 5912 covers: MCC 5912, drug stores and pharmacies.
How a rolling reserve ties up cash
A rolling reserve keeps a share of each day's sales and releases it after a set period. The cash it holds grows with your sales.
Example
You sell $100,000 a month with a 10% reserve held 90 days. Each month $10,000 is held. From month 4, the first month's $10,000 comes back, and about $30,000 sits in reserve at any time.
| Monthly sales | Reserve | Period | Cash held at steady state |
|---|---|---|---|
| $50,000 | 5% | 90 days | About $7,500 |
| $100,000 | 10% | 90 days | About $30,000 |
| $100,000 | 10% | 180 days | About $60,000 |
Simple arithmetic on the reserve terms; real releases follow your contract's schedule.

- $10,000Held until month 4Paid back month 4
- $10,000Held until month 5Paid back month 5
- $10,000Held until month 6Paid back month 6
Each month's 10% returns three months later.
If you bill refills monthly, a reserve also lags behind your subscriber base. Plan cash for it before a launch: payout timing for online pharmacies.
Reserves at Stripe and PayPal
Stripe lists online pharmacies as a restricted business and PayPal requires pre-approval for prescription items. If either approves you, its reserve rules apply.
| Provider | How reserves work |
|---|---|
| Stripe | Fixed or rolling; "a temporary hold on a portion of a business's funds for a predetermined period of time"; "in some rare cases" indefinite (Stripe)6Stripe Support, Reserves: frequently asked questions, read November 2, 2026 |
| PayPal | Rolling, minimum and jumpstart reserves; example: "10% and held for a 90-day rolling period"; funds can be held "for up to 180 days" (PayPal)7PayPal, The why and what of account reserves, November 17, 20258PayPal User Agreement, updated September 14, 2026 |
PayPal also warns that "you might never be able to fully remove a reserve from your account"7PayPal, The why and what of account reserves, November 17, 2025. Stripe's position in detail: does Stripe allow online pharmacies?

How Tagada helps
LegitScript included, and no refund on a declined prescription
If you're launching a brand that sells prescription treatment online, Tagada Rx puts storefront, doctors, pharmacy and payments in one stack. You bring the brand and the ads.
- Payments and LegitScript included: your store runs on a Tagada-hosted domain that holds the LegitScript certification
- Capture on approval: "The card is only captured once a doctor approves." If the doctor declines, the hold is released and the patient pays nothing, so there is no refund to process
- Rebill on ship day: recurring orders are charged the day the pharmacy ships, not on a calendar date
- Pharmacy routing to a licensed US pharmacy
- Open by application to a selected group of brands; Tagada reviews every application
Terms are on the Tagada Rx page. Already running checkout and routing yourself? See payment routing for online pharmacies.
