Why Your Online Pharmacy Gets Traffic but No Sales
Why an online pharmacy gets visits but no orders: tracking, trust signals visitors check, the prescription step, checkout friction and declines.
Article summary
TL;DR
Visitors to an online pharmacy arrive suspicious. The FDA tells them to check for a license, a US address, a pharmacist and a prescription requirement, and to leave if any is missing. Check your data first, then find the step where visitors drop off. The usual leaks are missing trust signals, a confusing prescription step and declined cards.
- The FDA counts roughly 35,000 active online pharmacies, of which about 5% comply with US law according to NABP (FDA figure, content dated September 2020).
- The FDA tells consumers to buy only from a pharmacy that requires a prescription, has a US address, a state license and a pharmacist available.
- Shoppers abandon about 70% of carts on average (Baymard, 50 studies, September 2025).
- 10% of US shoppers who abandoned a checkout cite a declined card (Baymard).
- Meta only runs online pharmacy ads for advertisers "actively certified with LegitScript".

This guide is part of our playbook on the best ecommerce platform for online pharmacies. The general guide, for any business: checkout optimization.
Step 0: check that the data is real
Before you change a page, make sure the sales are missing and not just unrecorded.
Compare orders in your back office with purchases in your analytics
A gap of more than a few percent means tracking, not conversion, is broken.
Check that the purchase event fires once
After payment, on every payment method.
Check orders that wait for review
If the card is only captured after approval, count authorized orders too, or your conversion rate will look far too low.
Exclude your own team and bots
From sessions.
Tracking setup: server-side tracking for online pharmacies.
Find where visitors drop off
Conversion is a chain. Measure each link before you guess.
| Step | What to measure | If it is low, look at |
|---|---|---|
| Landing to product page | Share of sessions that view a product | Traffic quality, page message |
| Product page to cart or start | Add-to-cart or "start" rate | Trust, price clarity, prescription explanation |
| Cart to checkout | Checkout starts | Extra costs, account creation |
| Checkout to payment | Payment attempts | Form length, prescription or intake step |
| Payment to order | Successful payments | Declines, 3D Secure, processor limits |

The step with the largest drop is your first fix.
The step with the largest drop is your first fix. A low rate everywhere usually points to traffic quality or trust.
Trust: visitors check if you are legitimate
People search "is this online pharmacy legit" before they buy. The FDA gives them a checklist, and they apply it to your site.
| What the FDA tells consumers1FDA, BeSafeRx frequently asked questions, content current as of September 21, 2020 | What to show on your site |
|---|---|
| "Requires a prescription from a licensed healthcare professional" | Say clearly, near the price, that a valid prescription is required |
| "Provides a physical address in the United States" | Address in the footer and on the contact page |
| "Is licensed by the state board of pharmacy in your state and the state where the pharmacy is operating" | License numbers by state, with links to each board's lookup |
| "Has a state-licensed pharmacist available to answer your questions" | A named way to reach a pharmacist: phone, chat or message |

The FDA also lists warning signs: sites that sell "without a prescription", offer "discounts or cheap prices that seem too good to be true", or ship "from a foreign country"1FDA, BeSafeRx frequently asked questions, content current as of September 21, 2020. A legitimate pharmacy that looks like one of these loses the sale.
- Be findable in the board's database. The FDA tells consumers: "If your online pharmacy is not listed, you should not use that pharmacy"2FDA, BeSafeRx, Locate a state-licensed online pharmacy, content current as of November 13, 2024.
- Show accreditation if you have it. NABP's Digital Pharmacy Accreditation or a LegitScript certification is something visitors can verify7NABP, Digital Pharmacy Accreditation, read November 2, 2026.
- Avoid "no prescription needed" wording anywhere, including ads and meta descriptions.
Traffic quality: do visitors match your offer?
Pharmacy ads are restricted, so many pharmacies lean on traffic that does not buy.
- Paid search and social need certification. Google requires LegitScript or NABP accreditation plus Google certification for online pharmacies in the US5Google Ads policy, Healthcare and medicines, read November 2, 2026; Meta requires LegitScript4Meta Advertising Standards, Drugs and pharmaceuticals, read November 2, 2026. Without it, traffic often comes from content that attracts readers, not patients.
- Informational traffic converts poorly. Visitors reading about a condition are not ready to order a refill.
- Out-of-area visitors cannot buy. If you are licensed in some states only, say so early.
Ads setup: Meta and TikTok ads for online pharmacies.
The prescription step: when and how you ask
The prescription step is where pharmacy checkouts lose the most clarity. Pick one model and explain it before the visitor starts.
| Model | How it works | Risk if unclear |
|---|---|---|
| Prescription first | Upload or transfer before paying | Visitors leave to find their paperwork |
| Pay, then verify | Card authorized at checkout, captured after approval | Visitors fear being charged for nothing |
| Clinician review | Intake, review by a clinician, then dispensing | Visitors do not know how long it takes or what it costs |
Example
For "pay, then verify", write next to the button: "Your card is authorized now and charged only if your prescription is approved." The Tagada Rx documentation adds that a hold "is not clinical approval" and the storefront should say so8Tagada docs, Tagada Rx introduction and how it works, read November 2, 2026.
Funnel structure: the online pharmacy sales funnel.
Price and checkout friction
- Show the full price early, including shipping and any review fee. Extra costs are the top reason for abandonment in Baymard's surveys3Baymard Institute, Cart abandonment rate statistics, updated September 22, 2025.
- Let customers check out as guests where your process allows it.
- Keep the form short. Ask only what the pharmacy needs at this step.
- Offer Apple Pay and Google Pay to skip card entry on mobile.
Details: checkout optimization for online pharmacies.
Payment declines: the cause nobody checks
A pharmacy can lose orders after the customer decided to buy.
- 10% of shoppers who abandoned a checkout cite a declined card (Baymard)3Baymard Institute, Cart abandonment rate statistics, updated September 22, 2025.
- Processors restrict pharmacies. Stripe lists "Online pharmacies" and "Card-not-present prescription-only products" as restricted businesses6Stripe, Prohibited and restricted businesses, last updated September 22, 2026, read November 2, 2026. A processor that limits your account can decline orders you never see.
- Check your decline rate by reason code every week. A spike in "do not honor" or fraud codes needs a different fix than expired cards.
Next steps: reducing payment declines for online pharmacies and payment routing and backup processors.
Follow up on the visitors who left
Most visitors who leave would still buy. Bring them back without exposing health information.
- Keep reminder emails and SMS generic: "You left something in your cart", never the name of a medication.
- Send one reminder within hours, one the next day, then stop.
- Answer the real objection: how the prescription works, the price, delivery time.

Symptom, cause, fix
| Symptom | Likely cause | First fix |
|---|---|---|
| Few product views from ads | Ad and page don't match | Align the ad promise with the landing page |
| Many product views, few starts | Missing trust signals | License, US address, pharmacist contact, prescription notice |
| Starts, then drop at the prescription step | Unclear process | Explain the model and timing before the step |
| Checkout drop at the total | Late extra costs | Show shipping and fees on the product page |
| Payment attempts, few orders | Declines | Read decline codes; add a backup processor for outages and timeouts |
| Orders in the back office, none in analytics | Broken tracking | Server-side purchase event |
