Online Pharmacy Benchmarks: Conversion Rate, AOV, CAC and Churn

What benchmarks exist for online pharmacies (few), the all-sector reference points worth using, and how to measure your own conversion, AOV and churn.

Article summary

TL;DR

As of November 2026, no source publishes conversion, order value, CAC or churn benchmarks for online pharmacies with a clear method and sample. Use all-sector ecommerce figures as reference points, say where they come from, and build your own baseline. In a pharmacy, count orders waiting for review and track the approval rate, or your numbers will mislead you.

  • Shoppers abandon about 70% of carts on average across 50 studies (Baymard, September 2025).
  • Failed payments cause 25% of lost subscriptions, according to Stripe (January 2024).
  • Post-purchase upsells average 4.7% acceptance across sectors (ReConvert, 2023 data).
  • The one "online pharmaceutical" panel we found tracks 15+ retailers with no published method. Don't benchmark against it.

This guide is part of our playbook on the best ecommerce platform for online pharmacies. The general guide, for any business: checkout optimization. Brands that want the prescription side run for them can apply to Tagada Rx, a stack with storefront, doctors, pharmacy and payments.

Why there is no online pharmacy benchmark

Online pharmacy data exists, but none of it is fit to compare against.

  1. Panels without a method

Grips Intelligence shows a conversion rate and AOV range for "online pharmaceutical", but on "15+ retailers tracked", with no country or method stated6Grips Intelligence, Online pharmaceutical industry page, September 2026 data, read November 2, 2026.

  2. Market reports, not store metrics

Market size studies tell you nothing about your checkout.

  3. Mixed models

A refill pharmacy, a clinician-led brand and an OTC catalog convert differently. One average would hide all three.

If you see a pharmacy conversion rate without a sample, date and method, treat it as marketing.

All-sector reference points

These figures come from all ecommerce sectors. They are ceilings and floors to test against, not pharmacy targets.

The metrics that differ in a pharmacy

A pharmacy funnel has steps a regular store doesn't: eligibility, prescription, review. Measure them, or the standard metrics break.

MetricHow to calculate itPharmacy trap
Conversion rateOrders (authorized + captured) ÷ sessionsCounting only captured orders understates conversion while reviews are pending
Approval rateApproved orders ÷ authorized ordersLow approval means the wrong visitors, not a bad checkout
AOVRevenue ÷ orders, split first order and refillA blended AOV hides the refill pattern
CACAd and sales spend ÷ new approved customersDividing by all checkouts flatters CAC
Refill retentionShare of plans reaching refill 2, 3 and 6Monthly churn alone hides the drop at the first refill
Payment recoveryRecovered renewals ÷ failed renewalsDeclines on refills look like churn

What to fix first

Fig. 4Fix in this order

  1. Tracking. Back-office orders and analytics must match before any benchmark means anything. See server-side tracking.
  2. Approval rate. If many orders are declined at review, fix targeting and the eligibility step before the checkout.
  3. Declines. Read refill decline codes weekly; set up retries. See failed payment recovery.
  4. First refill. Remind before the supply runs out. See subscriptions and churn.
  5. Order value. Only then work on add-ons: how to increase AOV.

Each step links to the guide that covers it.

Order value comes last.

Frequently asked questions

What is a good conversion rate for an online pharmacy?

No source publishes one with a method as of November 2026. Track yours as authorized plus captured orders divided by sessions, and compare month to month. All-sector data, such as Baymard's 70% average cart abandonment, gives a reference point, not a target.

What is the average order value of an online pharmacy?

There is no reliable published figure. Split your own AOV into first orders and refills: they behave differently, and a blended number hides your refill business.

How do I measure churn for prescription refills?

Track the share of refill plans that reach the second, third and sixth refill, and separate cancellations from failed payments. Stripe attributes 25% of lost subscriptions to failed payments across its data.

Which KPIs matter most for an online pharmacy?

Conversion including authorized orders, approval rate at review, CAC per approved customer, refill retention and payment recovery. The approval rate is the one regular stores don't have, and often the first to fix.
