Best Ecommerce Platform for Telehealth Companies

How to choose an ecommerce platform for a telehealth brand: processor rules, capture after doctor approval, refills, patient data and ads.

Article summary

TL;DR

The best platform for a telehealth company is the one that connects four things a normal store never has to: an intake quiz, a licensed clinician's decision, a pharmacy, and a card that is charged only after that decision. Pick it on payment rules, capture after approval, refills, patient data and ad eligibility. Store features come last.

  • Stripe lists "Telemedicine and telehealth services" as a restricted business: approval is case by case, and LegitScript is not required (read November 2, 2026).
  • Shopify Payments lists "prescription drugs" among products that need regulatory approval, and Shopify's own policy gives uploading HIPAA-covered health data as an example of illegal use.
  • Google Ads and Meta both require LegitScript certification before a telehealth provider can advertise in the US.
  • The FTC sued a large telehealth brand on July 29, 2026, alleging hidden subscription terms, hard cancellation and health data shared with ad platforms.

Why a telehealth brand chooses a platform differently

A skincare or apparel brand picks a platform for its themes and apps. A telehealth brand sells a consultation and a treatment plan, and that changes four things.

RiskWhat happensWhere it hits
Restricted processingStripe reviews telemedicine case by case and asks for license numbers and process details2Stripe Support, Prohibited and Restricted Business List FAQs, read November 2, 2026Your launch date, and your account if the offer changes
Charging before a decisionA card charged before a clinician approves means refunds, disputes and complaintsChargebacks and FTC exposure
Health data in commerce toolsStripe's contract bars sending Protected Health Information3Stripe Services Agreement, section 4 (Third Party Data), amended September 28, 2026; Shopify names HIPAA data uploads as an illegal use5Shopify Acceptable Use Policy, read November 2, 2026Your store, your email tool, your ad pixels
Ad eligibilityGoogle and Meta require LegitScript for telehealth providers8Google Ads policy, prescription drug services, read November 2, 20269Meta Advertising Standards, Drugs and pharmaceuticals, read November 2, 2026Your main acquisition channel

None of these appear in a feature checklist. All four decide whether the brand can launch and keep running.

Watch out

In the Hims & Hers case, the FTC alleges consumers were "charged for and subscribed to a prescription […] without having a chance to review or approve it", and that health information was shared "with third-party advertising platforms such as Meta and Snap"10FTC, press release on the complaint against Hims & Hers, July 29, 2026 (allegations; case pending). These are allegations in an open case, but they show where regulators look first.

Criterion 1 is covered in depth in payment routing and backup processors. Criteria 3 and 5 are in subscription billing for telehealth companies and memberships for telehealth companies.

Storefronts need the clinical layer added; a telehealth stack brings it.

From the table above, with the date each page was read.

Shopify and WooCommerce stay strong storefronts. The work is everything around them: the clinical layer, the capture rule and the refill logic. If you are weighing the two models, read Shopify or a sales funnel for your telehealth company.

Which setup fits your model

The right platform depends less on your size than on what the patient buys.

What you sellWhat you need mostSetup that fits
One-off consultationBooking, payment at checkout, simple receiptsA booking tool with a processor that accepts telemedicine
Treatment plan on subscriptionHold and capture after approval, ship-day refills, online cancellationA stack that links clinician decision, pharmacy and billing
Membership with ongoing careRecurring billing, patient inbox, dunningSubscription billing with a customer portal (memberships)
Existing storefront, adding careA clinical and payment layer behind your storeHeadless connection to a telehealth stack

Example

A brand selling a monthly treatment plan through Meta ads ranks criteria 2, 3 and 6 first. A brand selling a single consultation ranks criterion 1 first and can live with a simpler store.

How the payment flow should work

In telehealth, checkout is not the end of the sale. The clinician's decision is.

  1. Patient checks out. Quiz, consent, and a hold on the card, not a charge.
  2. Clinician reviews. A clinician licensed in the patient's state approves or declines.
  3. Approval captures, decline releases. On approval the hold is captured; on decline it is voided and the patient pays nothing.
  4. Pharmacy ships. The prescription is routed and the order ships.
  5. Refill bills on ship day. The next charge follows fulfillment, and declined or cancelled cases never rebill.

Fig. 3The sale ends at the clinician's decision

  1. Quiz and holdConsent, and a hold on the card, not a charge
  2. Clinician reviewsLicensed in the patient's state
  3. Capture or releaseApproval captures, decline voids the hold
  4. Pharmacy shipsPrescription routed, order shipped
  5. Rebill on ship dayDeclined or cancelled cases never rebill

The card is captured at step 3, not at checkout.

Whatever platform you choose, ask the vendor to show you steps 1 to 3 working. More on holds in credit card pre-authorization holds and on what processors check in telehealth payment processing.

Patient data, cancellation and ads: the compliance layer

Three rules shape the platform more than any feature.

Ad setup is detailed in Meta and TikTok ads for telehealth companies.

What to check before you commit

Run this list with any vendor. A clear "yes" on each point saves a migration later.

  1. Processor: which processors accept a telehealth consultation and a treatment plan on this platform?
  2. Capture: can the card be held at checkout and captured only after clinician approval?
  3. Refills: do renewals follow fulfillment, and stop on declined or cancelled cases?
  4. Data: where are intake answers stored, and does any clinical data reach the store, email or ad tools?
  5. Cancellation: can a patient cancel online, and is consent logged?
  6. Ads: is the domain LegitScript certified, and are purchase events sent server-side without health data?
  7. Exit: can I export patients' commerce data, orders and subscriptions?

A clear yes on each point saves a migration later.

Frequently asked questions

Can you use Shopify for a telehealth company?

Yes, as a storefront. Shopify Payments lists prescription drugs among regulated products, and Shopify's Acceptable Use Policy gives uploading HIPAA-covered health information as an example of illegal use. So intake, clinician review and prescriptions run in a separate clinical system, and you check which processor will accept your offer.

Do telehealth companies need a special payment processor?

Not always, but mainstream options are limited. Stripe lists telemedicine as restricted and reviews each business case by case. Square's US terms exclude pharmacy services where medication is fulfilled after an internet or phone consultation. Plan for a processor that has reviewed your exact model, and a backup. See telehealth payment processing.

Does a telehealth platform need to be HIPAA compliant?

The parts that handle clinical data do: intake answers, messaging, prescriptions. Payment processing alone is generally outside HIPAA, but processors still forbid receiving health data; Stripe's agreement bars sending Protected Health Information. Choose a platform where clinical data never reaches your store, payment or ad tools.

Do I need LegitScript to launch a telehealth brand?

Not to process payments with Stripe, whose FAQ says "Telemedicine businesses don't need certification from LegitScript". You need it to advertise: Google and Meta both require LegitScript certification for telehealth providers in the US, plus their own authorization. Certification applies to the domain your store runs on.

When should the patient's card be charged?

After a clinician approves. Hold the card at checkout and capture only on approval, then bill refills when the pharmacy ships. This avoids charging patients who are not prescribed, which is one of the practices the FTC alleges in its 2026 complaint against a telehealth brand.
