Shopify Checkout for Telehealth Companies
What a telehealth brand can do in Shopify checkout: payment rules, HIPAA, intake order, charging after the clinician decides, refills and tracking.
Article summary
TL;DR
Shopify works as a telehealth storefront, but its checkout was built for retail, not care. Three limits matter: health data shouldn't sit in Shopify, the card should be charged only after a clinician decides (a Shopify Payments authorization lasts 7 days), and Shopify allows one card processor at a time. Keep intake and clinical data outside Shopify.
- Shopify Payments lists "prescription drugs" among products that need "regulatory approval or compliance"; telemedicine is not named (read November 2, 2026).
- Shopify's Acceptable Use Policy gives "uploading Protected Health Information subject to HIPAA" as an example of illegal use; HIPAA Journal reports Shopify "will not sign a Business Associate Agreement".
- Shopify Payments gives "an authorization period of 7 days for credit card payments" when you capture manually.
- One card processor at a time; a third-party one costs 2% (Basic) down to 0.2% (Plus) per order.
- Shopify's terms, updated October 20, 2026, ask merchants not to "process orders outside Shopify's Checkout".

What Shopify's rules say about telehealth
Shopify doesn't ban telehealth. Its rules touch three things: the medication, health data and the checkout itself.
| Rule | What it says | What it means for you |
|---|---|---|
| Shopify Payments eligibility | "Products or services that require regulatory approval or compliance, such as […] prescription drugs, medical devices" (list "not exhaustive")1Shopify Help Center, Shopify Payments eligibility, read November 2, 2026 | Selling the medication through Shopify Payments needs approval; a consultation alone is not named |
| Acceptable Use Policy | Gives "uploading Protected Health Information subject to HIPAA" as an example of illegal use2Shopify Acceptable Use Policy, read November 2, 2026 | Keep intake answers and clinical data out of Shopify |
| Business associate agreement | Shopify "will not sign a Business Associate Agreement", as reported by HIPAA Journal (2023)3HIPAA Journal, Is Shopify HIPAA compliant?, November 2, 2023 (secondary source) | A covered entity can't treat Shopify as a vendor for health data |
| Terms of Service, section 1 | "You agree not to work around, bypass, or circumvent any of the technical limitations of the Services, including to process orders outside Shopify's Checkout."4Shopify Terms of Service, section 1 (Account Terms), updated October 20, 2026 | Read this before sending Shopify traffic to an outside checkout |
If the brand sells and ships the medication itself, the payment rules are those of an online pharmacy: see online pharmacy payment processing.
Intake before or after checkout?
On Shopify, the intake has to live in another tool. The real choice is the order of the steps.
| Order | How it works | Risk |
|---|---|---|
| Intake first, then Shopify checkout | The intake tool decides eligibility, then sends the patient to a Shopify cart | Health data and checkout live in two systems; you must link them without copying answers into Shopify |
| Shopify checkout first, then intake | Apps redirect the buyer to a medical intake after payment | The card is charged before anyone reviewed the case, unless you capture manually |
| Intake, card hold, review, capture | The card is authorized at checkout and charged only if a clinician approves | Needs a capture step tied to the clinical decision, within the authorization period |

- Shopify checkout first, then intakeAt checkout, before anyone reviewed the caseCharged before review, unless you capture manually
- Intake, card hold, review, captureOnly if a clinician approvesCapture tied to the decision, within the authorization period
Only the third waits for the clinician.
Charge only after the clinician decides
Charging before a decision means refunds for every declined patient, and disputes from those who didn't expect the charge. Shopify can authorize first and capture later:
Set the store to manual capture
In the payment settings, so checkouts only authorize the card5Shopify Help Center, Capturing payments for your orders, read November 2, 2026.
Capture within the authorization period
"Shopify Payments provides an authorization period of 7 days for credit card payments"5Shopify Help Center, Capturing payments for your orders, read November 2, 2026. Other providers vary.
Tie the capture to the clinical decision
Someone, or something, must capture approved orders and cancel declined ones, every day.
Say it at checkout
The patient should know the card is held and charged only after approval.
Watch out
If the review, an ID check or a missing intake takes longer than the authorization period, the hold can expire and you may not collect the payment5Shopify Help Center, Capturing payments for your orders, read November 2, 2026. Plan the review time inside 7 days, or a fresh payment request.
Payments and refills in Shopify checkout
- Shopify Payments first, if approved for your model1Shopify Help Center, Shopify Payments eligibility, read November 2, 2026. Its processors apply their own rules: Stripe, for example, reviews telemedicine case by case and asks for "license numbers and information on your processes"10Stripe Support, Prohibited and Restricted Businesses List FAQs, read November 2, 2026.
- Otherwise, one third-party card processor6Shopify Help Center, Configuring third-party payment providers, read November 2, 2026, with Shopify's fee on each order: 2% on Basic, 1% on Grow, 0.6% on Advanced, 0.2% on Plus7Shopify pricing, third-party payment provider fees, read October 9, 2026.
- No cascade inside Shopify checkout. With one card processor, an outage or a paused account stops card payments until you switch.
- Refills: Shopify Subscriptions works with Shopify Payments, PayPal Express, Authorize.net, Adyen and Stripe (Stripe "available only to select merchants")8Shopify Help Center, Shopify Subscriptions considerations, read October 9, 2026. A treatment plan billed on a calendar date can drift from the pharmacy's shipping date.

Routing between processors needs a layer outside Shopify's checkout: see payment routing and backup processors for telehealth companies. Refill billing is in subscription billing for telehealth companies.

Tracking after the script sunset
Script tags on Shopify's Thank you and Order status pages stopped on August 28, 2025 for Plus stores and on August 26, 2026 for other plans9Shopify.dev, Apps in checkout, read November 2, 2026. For a telehealth brand, that is a chance to clean up:
- List what used to run on the Thank you page; old scripts often sent product names to ad platforms.
- Rebuild browser tracking with web pixels supported by Shopify9Shopify.dev, Apps in checkout, read November 2, 2026, on general pages only.
- Send the purchase server-side as a generic event, with no treatment name.
- Compare paid orders with ad platform purchases for two weeks.
Rebuild tracking without product names.
What to send and not send: server-side tracking for telehealth companies.
Keep Shopify checkout, add a layer, or move the funnel?
| Your situation | Choice |
|---|---|
| Shopify Payments approved your model, no medication billed through Shopify | Keep Shopify checkout, intake in a separate tool, manual capture |
| Your own processor, no backup needed | Shopify checkout with one third-party processor, and its fee |
| You need hold-then-capture tied to the clinical decision, and refills on ship day | A telehealth checkout outside Shopify for the treatment funnel |
| You need a backup processor or routing | A checkout layer that supports several processors (check Shopify's terms first4Shopify Terms of Service, section 1 (Account Terms), updated October 20, 2026) |
| You want doctors, pharmacy and payments run for you | A full telehealth stack, with Shopify kept as a content site if you like |
