One-Click Upsells for Telehealth Companies
What a telehealth brand can upsell in one click after checkout, when to show it (before or after approval) and the rules for recurring offers.
Article summary
TL;DR
A one-click upsell is an offer shown right after payment that the patient accepts without re-entering their card. In telehealth, the clinician hasn't reviewed the case yet, so the upsell can't be a treatment. Offer a longer prepaid term, a membership, a follow-up visit or a non-prescription item, conditional on approval, with every recurring term shown.
- Across 40,000+ merchants, ReConvert reports a 4.7% average conversion rate for post-purchase upsells and 1.7% for thank-you page offers (2023 data, all sectors).
- Stripe lists "high-pressure upselling" among the practices it does not support (read November 2, 2026).
- The FTC alleges Hims & Hers enrolled consumers in treatment subscriptions "shortly after receiving consumers' intake forms" (complaint filed July 29, 2026).
- An upsell that adds a recurring charge needs clear terms and express consent under ROSCA.
- With Tagada Rx, "Declined, cancelled, and errored cases never rebill".

This guide is part of our playbook on the best ecommerce platform for telehealth companies. The general guide, for any store: one-click upsells. Upsells come after the order bump in the telehealth sales funnel.
Three moments to make an upsell
A telehealth order has three moments where the patient has already said yes to something. Each fits a different offer.
| Moment | What the patient has done | What you can offer | Condition |
|---|---|---|---|
| Right after checkout | Authorized payment for a consultation or plan | Longer prepaid term, membership, follow-up visit, non-prescription item | Charged only if the clinician approves the main order |
| After clinician approval | Received a treatment plan | Membership, prepaid term, support add-ons | The clinician's plan is not changed by the offer |
| At refill or renewal | Stayed on the plan | Switch to a longer term, add a non-prescription item | Same price disclosure and consent as a new sale |

- Refill or renewalLonger term, non-prescription item
Each yes opens a different offer.
In ReConvert's ecommerce data, offers shown right after payment convert better than thank-you page offers (4.7% vs. 1.7%)1Upsell.com (formerly ReConvert), The impact of post-purchase upselling in 2023, January 25, 2024 (data from 40,000+ merchants in 12 countries). In telehealth, that moment is also the one with the least clinical information, so keep the offer simple.

Upsell ideas that fit a telehealth brand
| Upsell | Why it fits | Watch for |
|---|---|---|
| Prepay 3 months instead of monthly | Fewer renewals to fail, a lower price per month | Release or refund it if the clinician declines; state the refund terms |
| Membership | Ongoing access to the care team, member pricing | Recurring terms in full on the upsell page |
| Follow-up visit at a set price | The patient knows the cost of the next step | The clinician decides what the visit covers |
| Non-prescription item from your store | Useful from day one, no clinical decision | NoHealth claims in the copy |
| Faster shipping or shipping protection | If not already offered as a bump | Don't sell it twice |
Upsells over memberships in detail: memberships for telehealth companies.
What not to upsell
A second treatment or a different medication
Only the clinician decides, after reviewing the case.
A “stronger” or “better” plan
That is a clinical claim, and a pressure tactic.
Faster clinical review
It ties money to the medical decision.
A chain of offers
Stripe lists "high-pressure upselling" among the practices it does not support2Stripe, Prohibited and restricted businesses, read November 2, 2026. One offer, with a clear "No thanks" link.
Watch out
The FTC's complaint alleges Hims & Hers charged consumers for and subscribed them to a prescription "without having a chance to review or approve it"3FTC, FTC and States Act Against Hims & Hers for Deceptive and Unlawful Privacy Practices, press release, July 29, 2026 (allegations; the case is ongoing). An upsell that adds treatment before review repeats the pattern the FTC is suing over.
Upsells that add a recurring charge
Prepaid terms and memberships are the most valuable upsells, and the most regulated.
- Disclose the terms on the upsell page: price, billing frequency, renewal date, how to cancel. ROSCA requires material terms before billing information is obtained4Restore Online Shoppers' Confidence Act, 15 U.S.C. § 8403.
- Get express consent with a button that names the commitment ("Add membership, $X per month").
- Confirm by email with the terms and the cancellation link.
- For an introductory price, Visa requires a notice before the full price starts (Visa Core Rules, April 2026)5Visa Core Rules and Visa Product and Service Rules, April 18, 2026, table 5-21.
- Let patients cancel online, as easily as they signed up.
Same disclosure and consent as a new sale.

Build the upsell page
- One offer, one sentence on what it is, the price and the saving.
- Two buttons: "Add to my order" and "No thanks". Never hide the skip link.
- No health claims and no condition named in the copy, the product name or the order line.
- Same card, no re-entry: the point of one click.
- Mobile first: the page must work on a phone without scrolling to find the buttons.
Measure your upsell
| Metric | Formula | What it tells you |
|---|---|---|
| Take rate | Upsells accepted ÷ upsells shown | Is the offer relevant? |
| Revenue per checkout | Upsell revenue ÷ checkouts | What it adds per patient |
| Refunds and cancellations on upsells | Refunded or cancelled ÷ accepted | Did patients understand it? |
| Disputes on upsell charges | Disputes ÷ accepted | Did patients recognize the charge? |
| 90-day retention of upsold patients | Active at 90 days ÷ upsold | Does it help or hurt retention? |
