How to Increase AOV for a Telehealth Company

Raise revenue per patient with multi-month plans, add-ons and post-approval upsells, without breaking subscription rules or clinical review.

Article summary

TL;DR

In telehealth, raise order value with the offer, not with pressure. Sell longer plans with clear renewal terms, add non-prescription products and services at checkout, and offer upsells only after a clinician approves. Anything that changes a prescription goes back to the clinician. Measure revenue per patient at 90 days, not only the first order.

  • Meta ad data published by Madgicx puts the median telemedicine account AOV at $48.59, with a middle 50% from $12.46 to $233.75 (read November 2026).
  • Post-purchase upsells convert at 4.7% on average across Shopify stores, and 1.7% on the thank-you page (ReConvert, 2023 data).
  • ROSCA requires clear terms before billing, express consent and a simple way to stop any recurring plan.
  • The FTC alleges a telehealth brand charged patients for prescriptions "without having a chance to review or approve" them (complaint, July 29, 2026).

This guide is part of our hub on the best ecommerce platform for telehealth companies. The general guide, for any business: order bumps.

What AOV means for a telehealth brand

AOV is a weaker signal in telehealth than in retail. One patient can pay for a consultation, a monthly plan or a 3-month plan, and each gives a different first order.

MeasureFormulaWhy it matters
First captured orderCaptured revenue ÷ approved first ordersWhat one acquisition brings in today
Revenue per patient at 90 daysCaptured revenue in 90 days ÷ new patientsIncludes refills, the main source of value
AOV by modelSame, split by consultation, monthly plan, multi-month planAvoids averaging different offers

Count only captured payments. A card hold that a clinician declines is not revenue. Context on reliable numbers: telehealth company benchmarks.

Multi-month plans: the biggest lever

A plan billed every 3 months instead of monthly triples the first order and cuts the number of renewals that can fail.

  1. Let the clinician decide the duration

The patient picks a plan; the clinician confirms it fits.

  2. Show the full price and renewal date

Before the card is entered, and get express consent4Restore Online Shoppers' Confidence Act, 15 U.S.C. § 8403.

  3. Plan the renewal reminder

California requires a notice before an automatic renewal changes price, and Visa requires a reminder before an introductory price ends5California Business and Professions Code § 176026Visa Core Rules and Visa Product and Service Rules, edition of April 18, 2026, table 5-21.

  4. Price the discount honestly

Show the per-month cost and the total, not just the per-month figure.

Watch out

A discount that only works if the patient cannot cancel is a liability, not a lever. Regulators focus on what the patient knew before paying4Restore Online Shoppers' Confidence Act, 15 U.S.C. § 84037FTC, press release on the complaint against Hims & Hers, July 29, 2026 (allegations; case pending). Keep cancellation online and simple.

Order bumps and add-ons that need no prescription

An order bump is a checkbox offer on the checkout page. In telehealth, it works for items that do not need a clinician.

  • A priority or follow-up consultation, priced separately.
  • Non-prescription products that fit your brand, sold without health claims.
  • Faster shipping on the first order.
  • A membership upgrade with clearly stated benefits and renewal terms.

Keep product names neutral. Treatment names in product titles end up in payment data and receipts, which processors ask you to avoid. Setup details: order bumps for telehealth companies and bundles and quantity breaks.

Post-purchase upsells after clinician approval

A one-click upsell normally fires right after payment. With a hold-then-capture flow, timing changes.

  1. Patient checks out. The card is held, not charged.
  2. Clinician reviews and approves or declines.
  3. On approval, capture the original order.
  4. Offer the upsell on the confirmation page or by email, with the stored card.
  5. If the upsell touches the prescription, it goes back to the clinician before any charge.

What not to do

Some AOV tactics from retail turn into complaints and disputes in telehealth.

  1. Pre-checked boxes

    For paid extras or subscriptions.

  2. Upsells that imply a medical benefit

    Or a better outcome.

  3. Charging for a plan the clinician has not approved

  4. Hiding the per-month cost

Of a multi-month plan or the renewal date.

  5. Countdown timers and fake scarcity

    On a care offer.

Frequently asked questions

What is a good AOV for a telehealth company?

There is no reliable telehealth average. Meta ad data published by Madgicx shows a median telemedicine account AOV of $48.59, with half of accounts between $12.46 and $233.75. The spread reflects different models. Compare your first captured order and 90-day revenue per patient against your own history, split by offer.

Is it ethical to upsell in telehealth?

Yes, when the extra is optional, priced clearly and does not touch the clinical decision. Non-prescription items, faster shipping or a membership are fine as offers. Anything that changes a prescription must go to the clinician first. Avoid pre-checked boxes and claims that an add-on improves outcomes.

Should a telehealth brand sell 3-month plans?

Often, yes. A longer plan raises the first order and reduces failed renewals. The clinician should confirm the duration, and the patient must see the full price, renewal date and how to cancel before paying, as ROSCA requires. Show both the total and the per-month cost.

When should I show a one-click upsell if the card is only held?

After the clinician approves and the original order is captured. Show it on the confirmation page or in the approval email, using the stored card. If the upsell relates to the treatment itself, send it back for clinical review before charging.

Do order bumps work in telehealth?

They can, for items that need no prescription: a follow-up consultation, a non-prescription product or faster shipping. Keep the price visible and the box unchecked by default. Across ecommerce, post-purchase offers convert at around 4.7%; test your own rate before relying on it.
