Shopify Checkout for Online Pharmacies

What an online pharmacy can and can't do in Shopify checkout in 2026: prescription step, one card processor, health data, refills and tracking.

Article summary

TL;DR

Shopify checkout was not built for prescriptions. Shopify Payments doesn't support prescription drugs, the checkout takes one card processor at a time, Shopify's policy doesn't support storing health information under HIPAA, and a prescription step inside checkout needs Shopify Plus. Many pharmacies keep Shopify for the catalog and run the prescription and payment flow somewhere built for it.

  • Shopify Payments lists "prescription drugs" among the regulated products it can't support (November 2026).
  • Shopify allows "only one credit card payment provider activated at a time"; a third-party one costs 2% on Basic down to 0.2% on Plus, per order.
  • Shopify's Acceptable Use Policy names "uploading Protected Health Information subject to HIPAA" as an activity it doesn't support.
  • Checkout UI extensions in the information, shipping and payment steps are Plus only, and Shopify Scripts stopped running on June 30, 2026.
  • Shopify's Terms of Service, updated October 20, 2026, ask you not to "process orders outside Shopify's Checkout".

Where the prescription step can go

There is no native prescription approval in Shopify checkout. A merchant who asked the Shopify Community for "a checkout app that requires prescription approval" in November 2024 got no working answer11Shopify Community, Seeking checkout app with prescription approval feature for pharmacy, November 28, 2024.

Refills on Shopify

  1. Shopify Subscriptions

Works with Shopify Payments, PayPal Express, Authorize.net, Adyen and Stripe, the last "available only to select merchants"9Shopify Help Center, Shopify Subscriptions considerations, read October 9, 2026. Shopify Payments is already excluded for you.

  2. Subscription apps

They have their own gateway lists; check that yours supports your processor.

  3. A refill is not a plain renewal

Each refill depends on a valid prescription, which a calendar-based renewal doesn't know about.

Tracking after the script sunset

  1. List what used to run in checkout.liquid and Additional scripts: they stopped on August 28, 2025 for Plus and August 26, 2026 for script tags on other plans6Shopify.dev, Apps in checkout, read November 2, 2026.
  2. Move browser tracking to web pixels, available on all plans except Starter5Shopify.dev, Checkout technologies and plan availability, read November 2, 2026.
  3. Keep pixels away from prescription steps and account pages.
  4. Send purchases server-side with the same event ID as the browser event12Meta for Developers, Deduplicate Pixel and server events, read November 2, 2026, and without product names.

Rebuild tracking without touching prescription pages.

Keep Shopify checkout, or move it?

Your situationChoice
You sell only non-prescription products on ShopifyShopify checkout, with Shopify Payments if your products fit its rules
Prescription orders, low volume, prescription collected after the orderShopify checkout with one third-party processor, PHI kept outside Shopify
You need the card held until the prescription is approvedA checkout built for authorize-then-capture
You need a backup processor or routingA checkout that supports several processors
Refills tied to shipments and prescription checksA billing system that bills on shipment, not on a calendar

Before sending Shopify traffic to an outside checkout, read Shopify's Terms of Service: "You agree not to work around, bypass, or circumvent any of the technical limitations of the Services, including to process orders outside Shopify's Checkout"8Shopify Terms of Service, section 1, updated October 20, 2026. Ask Shopify or your counsel how this applies.

A store that leaves Shopify and runs on another platform is no longer under these terms. The broader choice: Shopify or a sales funnel for your online pharmacy?

Frequently asked questions

Can I use Shopify Payments for an online pharmacy?

No. Shopify Payments lists "prescription drugs" among regulated products it can't support. You can add one third-party card processor that accepts pharmacies, and Shopify charges 0.2% to 2% per order on top of that processor's fees, depending on your plan.

Can customers upload a prescription in Shopify checkout?

Not natively. In-checkout extensions that could add a step are Plus only, and apps usually collect the file on the product page or cart. Shopify's policy doesn't support storing health information subject to HIPAA, so keep prescriptions in a system built for them.

Is Shopify HIPAA compliant?

Shopify's Acceptable Use Policy names "uploading Protected Health Information subject to HIPAA" as an activity it doesn't support. Plan your store so prescriptions, health answers and patient records live outside Shopify, and keep order data free of health details.

Can I use two payment processors in Shopify checkout?

Not for cards. Shopify allows "only one credit card payment provider activated at a time". A backup processor, or a cascade to another processor when one is down or times out, needs a checkout or payment layer outside Shopify's own.

Can I hold the payment until the prescription is approved on Shopify?

Shopify checkout has no built-in approval step, and the Shopify Community thread asking for one found no ready app. Pharmacies usually collect the prescription after the order, or move the prescription and payment flow to a checkout built to authorize first and capture after review.
