How to Require a Prescription Before Checkout (and Charge Only After Approval)

How a telehealth brand charges for treatment only after a clinician approves: authorization holds, capture or void, hold length, checkout wording.

Article summary

TL;DR

Authorize the card at checkout without capturing it, send the case to a clinician, then capture if they prescribe and void the authorization if they don't. The patient pays nothing unless treatment is approved. Fit the clinical review inside the authorization window, about 7 days for an online card payment at Stripe, and say the hold isn't an approval.

  • Visa allows up to 10 days between a customer-initiated online authorization and its processing; Stripe's window is 7 days for most networks.
  • At Stripe, canceling an uncaptured payment costs nothing, and cardholders can only dispute captured payments.
  • Square doesn't support businesses where medication is fulfilled after an internet or phone consultation without a physical visit (terms updated July 30, 2026).
  • On July 29, 2026, the FTC alleged a telehealth company charged consumers for a prescription "without having a chance to review or approve it".
  • Stripe restricts "Card-not-present prescription-only products and pharmaceuticals".

This guide is part of our hub on telehealth payment processing. The general guide, for any business: subscription payment processing, from first charge to every renewal. If you sell against a prescription the patient already has, that's the pharmacy side: online pharmacy payment processing.

How a prescription-gated checkout works

  1. State and eligibility first: the patient's state, then the intake questions.
  2. Checkout with an authorization: the card is approved for the amount and the funds are held, not taken2Stripe Docs, Place a hold on a payment method, read November 2, 2026.
  3. Clinician review: a clinician licensed in the patient's state reviews the case.
  4. Capture or void: capture if they prescribe; void the authorization if they don't.
  5. Fulfillment: the prescription goes to the pharmacy, which ships.
  6. Renewals: the next cycle is charged when the next shipment goes out.

Why charge only after approval

Charging for treatment before review creates disputes and draws regulators. The patient pays for something nobody has agreed to provide.

RiskWhat the source says
FTC enforcementThe FTC alleges consumers were "charged for and subscribed to a prescription […] without having a chance to review or approve it"; allegations in a pending case6FTC, press release on Hims & Hers, July 29, 2026 (allegations; case pending)
Processor policyStripe restricts "Card-not-present prescription-only products and pharmaceuticals"4Stripe, Restricted and prohibited businesses, read November 2, 2026
Unsupported at SquarePharmacies "where fulfillment of medication is performed with an internet or telephone consultation, absent a physical visit"5Square, General Terms of Service, section 3, updated July 30, 2026
DisputesA captured charge for a declined case is a refund or a dispute; an uncaptured authorization can't be disputed at Stripe3Stripe Docs, Best practices for preventing fraud, read November 2, 2026

A visit fee sold as such can still be charged up front, if the patient knows it pays for the consultation whatever the outcome. Splitting the visit and the medication: charging the consultation and the medication separately.

Watch out

If the window ends before the clinician decides, the authorization expires and you can't capture it2Stripe Docs, Place a hold on a payment method, read November 2, 2026. You'd need a new authorization, which can be declined. Size your clinician capacity to decide well inside 7 days, and void what you won't use: Visa expects unused authorizations to be reversed1Visa Core Rules and Visa Product and Service Rules, edition of April 18, 2026, table 5-12.

More on holds in general: credit card pre-authorization holds.

Identity and state checks before the prescription

Check who and where the patient is before a clinician reviews the case. It keeps unlicensed states and false identities out of your paid orders.

  1. State

Ask it first and stop states your clinicians don't cover: telehealth across state lines.

  2. Identity

When an offer requires it, ask for a government photo ID before the prescription. Tagada Rx does this through the patient portal for offers marked as requiring ID8Tagada Docs, Rx: patient portal, read November 2, 2026.

  3. Age

We found no federal age rule specific to telehealth; your clinical protocol and state rules decide.

  4. Card and identity together

3D Secure checks the card, the ID checks the patient: fraud prevention and 3D Secure.

Checkout wording

Example

"Your card will be authorized for $89 today. You'll only be charged if your clinician approves your treatment. If they don't, the authorization is canceled and you pay nothing. Your bank may show a pending line for a few days."

On Shopify and other platforms

Holding checkout until a prescription is approved isn't a standard feature on most store builders. In a November 2024 Shopify Community thread, no one confirmed an app that does it9Shopify Community, Seeking Checkout App with Prescription Approval Feature for Pharmacy, November 28, 2024. Brands usually run the intake and authorization in a dedicated funnel or a headless checkout. More: can you sell telehealth services on Shopify? and checkout optimization for telehealth companies.

How Tagada helps

The payment layer behind a prescription gate

For the payment side of a gated checkout, TagadaPay brings "enhanced review for regulated verticals"11TagadaPay, homepage, read November 2, 2026 and "native 3DS2, device intelligence, and rules engine: approve real customers, block the rest".

  • Routing across TagadaPay and processors such as Stripe, NMI and Airwallex, with a cascade to another processor when one is down or times out
  • Your vault, portable: "Switch processors without re-onboarding customers"
  • Integrations: "Already on Stripe, NMI or Airwallex? We orchestrate those too."

Building a telehealth brand with the gate already built in? With Tagada Rx, "Approve captures the hold. Decline voids it. Money never leaves the card if the clinician does not prescribe"7Tagada Docs, Rx: introduction, read November 2, 2026, "Independent clinicians review every case before any charge", and the same stack plugs in headless through the SDK10Tagada, Tagada Rx, read November 2, 2026. US patients only, by selection.

Tagada follows Visa and Mastercard rules and the laws that apply to payments.

Frequently asked questions

Can I charge a patient only after the doctor approves the prescription?

Yes. Authorize the card at checkout, then capture after approval or void the authorization if the clinician declines. At Stripe, canceling an uncaptured payment costs nothing and the patient never sees a charge, only a pending line until the issuer releases it.

How long can I hold a card before the clinician decides?

Plan on 7 days for an online card payment at Stripe. Visa allows up to 10 days for a customer-initiated online authorization. If the window passes, the authorization expires and you need a new one.

Can a patient dispute an authorization hold?

Not while it's uncaptured: Stripe states cardholders can only dispute captured payments. Once you capture, the payment can be disputed like any other. Clear wording about the hold reduces "I don't recognize this" contacts.

Should I charge the consultation fee up front?

You can, if it's sold as a visit fee and the patient knows it's due whatever the outcome. Charging for treatment before review is what regulators look at. Keep the two amounts separate at checkout.

How is this different from an online pharmacy checkout?

A pharmacy checks a prescription the patient already has before dispensing. A telehealth brand creates the prescription through its own clinician review, so the payment waits on that decision. The pharmacy side: online pharmacy payment processing.
