Payout Timing for Telehealth Companies

Payout timing for telehealth companies: card holds until a clinician approves, authorization windows, settlement times, reserves and risk pauses.

Article summary

TL;DR

For most telehealth brands, the clock starts at capture, not at checkout. If you hold the card until a clinician approves, nothing settles until then, and the hold expires after about 7 days. After capture, card sales usually reach your bank in 2 to 3 business days. First payouts, reserves and risk reviews add time.

  • Stripe lists Visa card-not-present authorizations as valid 7 days for customer-initiated payments and 5 days for merchant-initiated ones (read November 2, 2026).
  • If an authorization expires before capture, "the funds are released and the payment status changes to canceled".
  • Stripe's US settlement timing is 2 business days; its first payout is "typically" scheduled within 7 to 14 days.
  • When Stripe pauses payouts for unacceptable risk, it's "typically" for 120 days from the first notice.
  • Stripe's approval for telemedicine "may be modified or revoked by Stripe at any time".

This guide is part of our hub on telehealth payment processing. The general guide, for any business: payout schedules and settlement times.

Telehealth payouts start at capture, not checkout

Many telehealth brands authorize the card at checkout and capture only after a clinician approves the prescription. That protects patients and reduces disputes, but it shifts your cash by the length of the review.

  1. Checkout: the card is authorized; funds are held on the patient's card, not paid to you.
  2. Clinical review: a clinician approves or declines.
  3. Capture on approval: the sale is now settled toward your balance. On a decline, the hold is released1Stripe Docs, Place a hold on a payment method, read November 2, 2026.
  4. Settlement: about 2 business days later in the US on Stripe2Stripe Docs, Payouts, read October 27, 2026.
  5. Payout to your bank: on your schedule, daily, weekly or manual2Stripe Docs, Payouts, read October 27, 2026.

Watch out

If reviews take longer than the authorization window, the hold lapses and you'd need to charge the card again. Track your review times, and release holds you won't capture.

Most of them depend on volume, disputes and terms.

Most of these are under your control: steady volume, few disputes, clear terms. Disputes are the biggest lever: see telehealth chargebacks.

Plan your cash around payouts

Telehealth pays some costs before patient money settles. Bask Health notes that pharmacy partners may need to be paid "before patient billing fully settles"8Bask Health, Healthcare cash flow risk: modeling liquidity exposure in subscription telehealth, February 25, 2026 (secondary source).

  1. Pharmacy and clinician fees

Check their payment terms against your capture and payout dates.

  2. Refund reserve

  3. Ad spend

If your cards pay for ads daily and payouts come in 2 to 9 days, fund the gap.

  4. Reserve

If your processor holds a share of each sale, model it in your forecast: see telehealth fees and reserves.

  5. A buffer for a pause

Frequently asked questions

How long do telehealth payouts take?

After capture, typically 2 business days on Stripe in the US, 2 or 3 on Adyen, up to 5 on Shopify Payments plus bank time. If you capture only after clinical approval, add the review time. First payouts take 7 to 14 days on Stripe.

How long can I hold a patient's card before capturing?

On Stripe, online card authorizations are generally valid 7 days, and 5 days for Visa merchant-initiated payments. After that, "the funds are released and the payment status changes to canceled". Extended authorizations exist for some eligible payments.

Why is my telehealth payout on hold?

Usually a first-payout delay, a reserve, a verification request or a risk review. Telemedicine approval on Stripe is service-specific and revocable, so new offers or a sudden volume jump can trigger a review. Answer verification requests fast.

Do refunds delay my payouts?

They reduce your balance, and if refunds exceed new sales, payouts stop until the balance recovers; Stripe can also debit your bank. Plan for refunds on declined or cancelled cases in your cash forecast.
