Chargeback Alerts for Telehealth Companies

How Ethoca, Verifi RDR and Order Insight keep telehealth disputes out of your ratio, what data to share with banks, and RDR rules by case.

Article summary

TL;DR

Chargeback alerts warn you that a patient is disputing a charge before it becomes a chargeback, so you can refund or answer first. Verifi (Visa) runs RDR and Order Insight; Ethoca (Mastercard) runs Ethoca Alerts and Consumer Clarity. Resolved alerts don't count in your dispute rate. For telehealth, what you send the bank must stay non-clinical.

  • Stripe: disputes resolved through RDR or Ethoca Alerts "don't count towards your overall dispute rates" (read November 2, 2026).
  • Visa's VAMP ratio excludes disputes resolved through a pre-dispute solution; the US merchant "Excessive" threshold is 1.5% since April 1, 2026.
  • In August 2026, Bloomberg reported that Visa placed a large US telehealth company in its monitoring program after subscription disputes rose in July.
  • Order Insight shares product descriptions with issuers "and ultimately with cardholders": keep them free of anything medical.
  • One alert vendor lists $15 per RDR or CDRN alert and $29 per Ethoca alert in the US (July 24, 2026).

Why telehealth disputes are different

Most telehealth disputes come from billing the patient didn't expect, not from stolen cards. A consultation, a treatment plan on subscription and a membership each create their own dispute pattern.

SituationLikely dispute reasonBest first tool
Renewal charged after the patient stopped using the planCancelled recurring, "not authorized"RDR rule, renewal reminders

The stakes are real. In August 2026, Bloomberg reported that Visa had placed Hims & Hers in its Acquirer Monitoring Program after disputes on subscriptions rose in July. To exit, the company must keep its dispute rate below 1.5% for three consecutive months, per the report4Subscription Insider, "Hims & Hers Faces Visa Monitoring Over Subscription Disputes", August 26, 2026, reporting a Bloomberg article of August 21, 2026 (secondary source).

Illustrative example.

Visa counts fraud reports and disputes together, on card-absent sales, and only flags merchants with at least 1,500 of them a month2Visa, Visa Acquirer Monitoring Program fact sheet, 2025. Mastercard's ECM program starts at 1.5% and 100 chargebacks, as reported by Chargeback Gurus9Chargeback Gurus, Understanding the Mastercard Excessive Chargeback Program, October 21, 2025 (secondary source). Ethoca Alerts help merchants exit ECM, HECM and EFM, per Stripe1Stripe Docs, How dispute prevention works, read November 2, 2026.

Tagada follows Visa and Mastercard rules and the laws that apply to payments.

Order Insight without sharing health data

Order Insight works by sending the issuer your order details: product description, amounts, delivery address, IP address, email, cancellation terms1Stripe Docs, How dispute prevention works, read November 2, 2026. Stripe says that by using it, "you direct Stripe to share this data with issuers and ultimately with cardholders"1Stripe Docs, How dispute prevention works, read November 2, 2026.

For a telehealth brand, that changes how you name things:

  1. Product description

"Monthly care plan" or "Online consultation", never a condition, a diagnosis or a medication name.

  2. Descriptor and store name

Your brand, recognizable to the patient, with nothing clinical.

  3. Metadata

Order IDs and plan names only. Stripe's agreement says users "must not provide Protected Health Information to Stripe"5Stripe Services Agreement, section 4 (Third Party Data), amended September 28, 2026.

  4. Terms field

A short summary of how to cancel, so the bank sees it too1Stripe Docs, How dispute prevention works, read November 2, 2026.

Watch out

CE 3.0 blocks only work when every payment carries an IP address, an email and a product description1Stripe Docs, How dispute prevention works, read November 2, 2026. A neutral description still counts: it just needs to be complete and consistent across orders.

With CE 3.0, if at least two earlier orders from the same cardholder share the IP address and at least one email or delivery address, the issuer must block a Visa "10.4 Other Fraud, Card Absent" dispute. It's never filed, and your rate doesn't move1Stripe Docs, How dispute prevention works, read November 2, 2026.

That fits refill patients well: they buy from the same device and address every cycle.

RDR rules for a telehealth brand

A broad RDR rule refunds disputes you could have won. Start narrow.

CaseSuggested ruleWhy
Renewal disputed as "cancelled recurring"Auto-resolveHard to win; the patient has left
Low amounts, such as a single consultation feeAuto-resolveFighting costs more than the refund. Stripe's example: potential fraud disputes under 10 USD1Stripe Docs, How dispute prevention works, read November 2, 2026
Fraud claim from a repeat patient, same IP and emailDon't auto-resolveOrder Insight with CE 3.0 can block it1Stripe Docs, How dispute prevention works, read November 2, 2026
First order shipped, high amountReview manuallyShipping proof and consent records may win it
Partial disputes or already-refunded paymentsNot covered by RDRVerifi doesn't support them; handle manually1Stripe Docs, How dispute prevention works, read November 2, 2026

Rules only apply to disputes started after you enroll. Earlier ones still arrive as chargebacks1Stripe Docs, How dispute prevention works, read November 2, 2026.

What alerts cost

Prices vary by reseller and processor. CDRN and Ethoca can both fire on the same payment: deduplicate by order so you don't pay twice. And every resolved alert is also a refund.

Set up in order

  1. Fix the causes: visible plan terms, a reminder before a trial or promo ends (Visa requires one at least 7 days before)7Visa Core Rules and Visa Product and Service Rules, edition of April 18, 2026, table 5-21, online cancellation. See cancellation compliance for telehealth.
  2. Clean your data: neutral product descriptions, a recognizable descriptor, IP and email on every payment.
  3. Turn on Order Insight and Consumer Clarity so banks see the order first.
  4. Add RDR with narrow rules for renewals and low amounts.
  5. Add Ethoca Alerts for Mastercard.
  6. Review monthly: alerts by plan, offer and reason; widen or narrow rules.

Prevention first, then alerts from the narrowest rules to the widest.

Failed renewals that the patient never meant to stop belong in recovery, not refunds: failed payment recovery for telehealth companies.

How Tagada helps

Fewer telehealth disputes before any alert fires

TagadaPay brings "native 3DS2, device intelligence, and rules engine: approve real customers, block the rest"10TagadaPay, homepage, read November 2, 2026, with "enhanced review for regulated verticals". Fewer disputes reach the alert stage:

  • Fraud filters before the charge: 3DS2 and rules that stop card testing before it turns into disputes
  • Dunning, email and SMS fired by real events, so patients hear from you before their bank
  • Your vault, portable: "Switch processors without re-onboarding customers"

Building the whole telehealth brand? With Tagada Rx, "the card is only captured once a doctor approves", recurring treatments rebill "the day the pharmacy ships", and "declined, cancelled, and errored cases never rebill"8Tagada Rx documentation, How it works, read November 2, 2026. US patients only, by selection.

Frequently asked questions

Do chargeback alerts lower my chargeback ratio?

Yes, when the dispute is resolved at the alert stage. Stripe says disputes resolved through RDR or Ethoca Alerts don't count towards dispute rates, and Visa's VAMP ratio excludes disputes resolved through a pre-dispute solution. Disputes filed before you enrolled still arrive as chargebacks.

What's the difference between Ethoca and Verifi?

Ethoca is a Mastercard solution, Verifi a Visa one. Verifi offers RDR, which resolves Visa disputes under your rules, CDRN alerts and Order Insight. Ethoca offers Ethoca Alerts and Consumer Clarity for Mastercard. Most telehealth brands need both networks covered.

Can I use Order Insight without sharing patient data?

Yes, if what you send is non-clinical. Order Insight shares product descriptions, amounts and addresses with the issuer and the cardholder. Use plan or service names, never a condition or medication, and keep health information out of payment data, as Stripe's agreement requires.

Are chargeback alerts worth it for a telehealth company?

Usually, once subscriptions drive renewals. Each alert costs a fee, $15 to $29 at one vendor, plus the refund, but keeps the dispute out of your ratio. Fix plan terms, reminders and cancellation first, or you'll pay for alerts you could have avoided.

What happens if my telehealth brand enters Visa's monitoring program?

Your processor is notified and you pay fees per dispute, $8 at the "Excessive" level as reported. In the case Bloomberg reported in August 2026, the brand had to stay below 1.5% for three consecutive months to exit. Alerts help bring the rate down.
